Ein Leberunterstützungsverfahren sollte die drei Hauptfunktionen der Leber – Detoxifikation, Regulation und Synthese – adressieren. Die Überlegung, dass für die vielfältigen klinischen Symptome beim Leberversagen vorwiegend die mangelnde Entgiftungsfunktion der Leber und somit der zunehmende Anfall von Toxinen im Körper verantwortlich ist, führte zur Entwicklung von Filtrations- und Adsorptionssystemen. Derartige entgiftende Systeme werden als artifizielle Leberunterstützungssysteme bzw. Detoxifikationssysteme bezeichnet. Komplexe regulative Prozesse und insbesondere die Synthese von lebenswichtigen Substanzen vermögen artifizielle Leberunterstützungssysteme nicht zu leisten. Für diese Funktionen werden Leberzellen benötigt. Extrakorporale Systeme, die diese biologische Komponente extrakorporal bereitstellen, werden als bioartifizielle Leberunterstützungs–systeme bezeichnet. Bei der Transplantation von isolierten Hepatozyten handelt es sich dagegen um eine rein biologische Unterstützungstherapie. Im Rahmen der vorliegenden Arbeiten wurden folgende Aspekte innovativer Strategien zur Therapie des Leberversagens adressiert: 1\. Weiterentwicklung und Evaluation eines einfachen extrakorporalen Detoxifikationskonzepts (Single Pass Albumin Dialysis [SPAD]), 2\. In vitro und klinische in vivo Evaluation des Modular Extracorporeal Liver Support (MELS) Systems, basierend auf SPAD als artifizielle und Leberzellen im CellModule Bioreaktor als bioartifizielle Komponente, 3\. Entwicklung und Evaluation des hohlfaserbasierten SlideReactor-Systems als Werkzeug für Zellkulturexperimente unter kontrollierten Bedingungen und der Möglichkeit der kontinuierlichen (Zeitraffer-)videomikroskopischen Beobachtung , 4\. Entwicklung von Methoden zur Isolierung und Kryokonservierung primärer humaner Leberzellen, 5\. Entwicklung einer Methode zur effizienten Markierung und Detektion primärer humaner Hepatozyten mittels eisenbasierter Partikel und MRT.Liver failure remains a life-threatening syndrome. With the growing disparity between the number of suitable donor organs and patients waiting for transplantation, efforts have been made to optimize the allocation of organs, to find alternatives to cadaveric liver transplantation and to develop extracorporeal methods to support or replace the function of the failing organ. An extracorporeal liver support system has to provide the main functions of the liver: detoxification, synthesis and regulation. The critical issue of the clinical syndrome in liver failure is understood to be the accumulation of toxins not cleared by the failing liver. Based on this hypothesis, the removal of lipophilic, albumin-bound substances such as bilirubin, bile acids, metabolites of aromatic amino acids, medium-chain fatty acids and cytokines should be beneficial to the clinical course of a patient in liver failure. This theory led to the development of artificial filtration and adsorption devices (artificial liver support). The complex tasks of regulation and synthesis remain to be addressed by the use of liver cells (bioartificial liver support). Hepatocyte transplantation is a solely biological approach to support the failing liver. The publications of the professorial dissertation address the following aspects of innovative treatment strategies in liver failure: 1\. Further development and evaluation of a simple extracorporeal detoxification concept (Single Pass Albumin Dialysis [SPAD]), 2\. In vitro and clinical in vivo evaluation of the Modular Extracorporeal Liver Support (MELS) system, based on SPAD as artificial und primary liver cells in the CellModule bioreactor as bioartificial components, 3\. Development and evaluation of the hollow-fiber based SlideReactor system as a tool for cell culture experiments under controlled conditions and continuous (time lapse) video-microscopic observation, 4\. Development of improved methods for isolation and cryopreservation of primary human liver cells, 5\. Development of an efficient labelling technique for detection of primary human hepatocytes after their transplantation using micron sized iron oxide particles (MPIO) and magnet resonance imaging (MRI)
