Deep brain stimulation (DBS) is an established and highly efficacious treatment option in idiopathic Parkinson’s disease, dystonia and essential tremor. In the past, the clinical effects of DBS were explained as local effects within the target structure of the DBS electrode. Nowadays, this concept is only considered part of the explanation of the mechanism of action of DBS. Rather, the concept has been extended by additive effects mediated via structural and functional connectivity of the target area to different other areas of the whole brain, namely the so-called cortex basal ganglia cerebellar loop.
