Comparing study of two methods of Cesarean section in consideration of the long term results of the subjective well-being of the patients

Die modifizierte Sektiotechnik nach Misgav-Ladach findet weltweit eine immer häufigere Anwendung in der Geburtsmedizin. In der vorliegenden proskektiven randomisierten Studie wurden Unterschiede zwischen der modifizierten Misgav- Ladach-Technik und der konventionellen Sektiotechnik im Hinblick auf die peri- und postoperative Verläufe als auch das subjektive Wohlbefinden der Patientinnen untersucht. Es wurden 126 Patientinnen in die Untersuchung eingeschlossen, 81 Patientinnen wurden nach der modifizierten Misgav-Ladach- Technik, 45 Patientinnen konventionell operiert. Die verkürzte Operationszeit sowie die Einsparung von Nahtmaterial kann als Vorteil der Misgav-Ladach- Methode gewertet werden. Unsere Untersuchung hat gezeigt, dass es zu keinen höheren postoperativen Komplikationsraten im Vergleich zu konventionell operierten Patientinnen kam. Eine geringere Morbidität bezüglich geringerer Schmerzen, weniger Kreislaufdysregulationen, raschere Mobilisierung wie sie in vielen anderen Untersuchungen propagiert wurden, konnten wir nicht feststellen. Ebenfalls ergaben sich keine Vorteile bezüglich des postpartalen stationären Aufenthaltes und des Entlassungs-Hb-Wertes. Interessant ist der Aspekt, dass bei Patientinnen, welche nach der Misgav-Ladach-Methode operiert worden waren, in den ersten Tagen postpartum weniger Probleme bei der Miktion auftraten.The modified section technique by Misgav-Ladach is used worldwide more often in obestetrics. In this prospective, radomized study the difference between the modified Misgav-Labach-techenique and the convetional sectio technique were analysed with regards to the peri- and postoperative progress as well as the subjective well-being of the patients. 126 patients were included in the study, 81 patients were operated by modified Misgav-Ladach, 45 patients recieved conventional operation. The shortened operating time and the reduction of suture can be assessed as an advantage. Our study showed no higher rate of postoperative complications compared with the convetional operated patients. A lower morbidity regarnding less pain, less circulatory dysfunction, fast mobilisation, as many investigator puplisied, could not be observed. There were even no advantage in the postpartal hospitalisation and the level of haemoglobin at the time of discharge. An interesting aspect is the fact, that patients operted by the Misgav-Ladach-technique, had less problems with the miction within the first days postpartum
