Comparing trimethoprim-sulfamethoxazole and ciprofloxacin in the empirical pharmacotherapy of acute non-complicated urinary tract ınfections in primary care

Amaç: Akut komplikasyonsuz idrar yolu enfeksiyonlarının ampirik tedavisinde kullanılmakta olan trimetoprim-sulfometoksazol (TMP-SM) ile siprofloksasin (SPF)’in birinci basamakta kullanımının karşılaştırılması. Gereç ve Yöntem: 1 Ekim- 21 Aralık 2001 tarihleri arasında Karasu Devlet Hastanesi Aile Hekimliği Polikliniğime dizüri gibi idrar yakınmalarıyla başvuran, eşlik eden bir başka hastalığı olmayan, yaşları 15- 78 arasında değişen, santrifüj edilmiş idrarın mikroskopisinde her sahada en az 5 lökosit saptanan 120 hastanın 60’ma randomize biçimde TMP-SM (160/800 mg/gün), 60Tna SPF (500 mg/gün) reçete edildi. Her iki ilaç için, tedavi öncesinde ve sonrasında idrardaki lökosit sayılarının ortalamaları karşılaştırıldı. Bulgular: Hastaların 82?si (%68,3) kadın, 38’i (%31,7) erkekti. TMP-SM kullanılan 60 hastanın 42’si (%70,0) kadın, 18’i (%30,0) erkek, SPF kullanılan 60 hastanın 40’ı (%66,7) kadın, 20’si (%33,3) erkekti. Hastaların yaş orta-laması 40,1 ± 15,4 (15- 78) idi. TMP-SM ve SPF verilen grupların ortalama yaşları sırasıyla, 39,3 ± 17,2 (15- 78) ve 40,8 ± 13,6 yıl (20- 73) idi. Tüm grup için, tedavi öncesi idrarda lökosit sayısı 11,28 ± 0,36 (6- 20) ve tedaviden sonraki lökosit sayısı 3,37± 0,19 (2- 10) olarak saptandı. TMP grubunun tedavi öncesi lökosit ortalaması 11,23 ± 0,54 (6- 20) ve SPF grubunun tedavi öncesi lökosit ortalaması 11,33± 0,47 (7- 20) bulundu. TMP grubunun tedavi sonrası lökosit ortalaması 3,30 ± 0,26 (2- 10), SPF grubunun tedavi sonrası lökosit sayısı ortalaması 3,45 ± 0,27 (2- 10) olarak saptandı. Her iki ilaç grubunda ilk ve son lökosit sayıları arasında anlamlı bir fark yoktu (p=0,57 ve p=0,89).Aim: To compare trimethoprim­ sulfamethoxazole (TMP-SMX) with ciprofloxacin for empiri­ cal pharmacotherapy ofnon-complicated urinary tract infection. Methods: The study was performed in 120 patients with complaints like dysuria of Karasu State Hospital Family Medicine Outpatient Clinic during October 1st -Dccember 21st 2001. The patients without any concomitant disease and 15- 78 years old with 5 white blood cells in the centrifuged urine were random divided into two groups. Sixty patients were prescribed TMP-SMX 160/800mg/day, the others cipro­ floxacin 500nlg/day. All the patients were re-checked after one week. The white blood cell counts in the urine before and after the' therapy were corn pared. Results: Eighty-two (68.3%) were female and 38 (31.70/0) male. There were 42 (70.00/0) female, 18 (30.00/0) male cases in TMP-SMX and 40 (66.7%) female, 20 (33.30/0)male cases in ciprofloxacin group. For all patients, mean age was 40.1 ± 15.4 (15-78). Mean ages were 39.3 ± 17.2 (15-78) and 40.8 ± 13.6 (20-73) for the TMP-SMX and ciprofloxacin groups, respectively. Urine white blood cells were 11.23 ± 0.54 (6-20) and 3.30±0.26 (2-10) in TMP-SMX, 11.33 ± 0.47 and 3.45 ± 0.27 (2-10) in ciprot1oxacin group, before and after the therapy. There was no statistically significant difference in the white blood cell count between the groups before (p=0.57) and after (p=0.89) the therapy. Conclusion: Because no significant difference in the clinical effectiveness was identified between TMP-SMX and ciprofloxacin in the non-complicated urinary tract infections in the primary care, TMP-SMX still seems first choice
