The role of follicular stem cells in pathophysiology of alopecia areata

Otkriće matičnih stanica u izbočini vanjske ovojnice korijena dlake potaknulo je istraživanja o mogućnosti korištenja folikula dlake kao terapijskog izvora matičnih stanica u regenerativnoj medicini. Folikuli dlake sadrže multipotentne matične stanice koje nakon indukcije i diferencijacije mogu dati specijalizirane stanice epidermisa, dlačnih folikula i pripadajuće lojne žlijezde, a sudjeluju i u regeneraciji epidermisa uslijed tkivnih ozljeda ili bolesti. Smještaj ovih matičnih stanica u izbočini vanjske ovojnice korijena dlake daje općenito objašnjenje zašto pojedini upalni procesi poput lichen planopilaris ili diskoidnog eritemskog lupusa uzrokuju trajan gubitak vlasi i/ili dlaka, nasuprot upalnim procesima poput alopecije areate, u kojima dolazi do regeneracije zahvaćenih folikula i ponovnoga rasta vlasi i/ili dlaka. Izbočina dlačnog folikula najprije je opisana u miševa, gdje tvori dobro diferenciranu strukturu. Nasuprot tome, nedostanak jasne morfologije izbočine dlačnih folikula u ljudi pridonio je sporijem tijeku istraživanja na folikularnim matičnim stanicama. Istraživanje matičnih stanica u folikulima dlake započelo je otkrićem mirnih stanica u folikulu, nazvanih stanicama koje zadržavaju biljeg (engl. label-retaining cells, LRC). No pravi zamah u istraživanju folikularnih matičnih stanica počeo je tek otkrićem i identifikacijom njihovih molekularnih biljega, poput keratina 15 (K15) i molekula CD34 u miševa te CD200 u ljudi, a koji su omogućili njihovu preciznu izolaciju iz tkiva, kultivaciju in vitro i utvrđivanje karakteristika i potencijala folikularnih matičnih stanica. Uloga folikularnih matičnih stanica u patogenezi alopecije areate danas ostaje nerazjašnjena. Zna se kako su oštećenja matičnih stanica rezultat upalnih procesa, a dlačni folikuli mjesta autoimunih reakcija. Ovome pridonose i drugi faktori poput gubitka imunološke privilegije folikula kao i autoimmune reakcije koje su rezultat okolišnih čimbenika i genetske predispozicije.The discovery of stem cells in the bulge region of the outer root sheath of hair follicles (HF), both in mice and man, has encouraged research into utilizing the hair follicle as a therapeutic source of stem cells for regenerative medicine. The hair follicle contains multipotent stem cells that can form epidermis, hair follicles and sebaceous glands as well as help in repopulation of epidermis after injury. Localization of these stem cells in the bulge area of the hair follicle may explain why some types of inflammatory alopecia cuase permanent loss of hair (scarring alopecia such as lichen planopilaris), while others (such as alopecia areata) are reversible (non-scarring alopecia). While being clearly distinguishable structure whitin hair follicles in mice, the lack of distinctive bulge morphology in human hair follicles has interefered studies of bulge cells. The study of the hair follicle stem cells started by identification of follicular stem cells in HF bulge as quiescent label retaining cells. The research of these cells emerged rapidly after the identification of bulge cell molecular markers, such as keratin 15 (K15) and CD34 in mice and CD200 in humans, which in turn allowed the isolation and further study of bulge cells from follicles. The pathogenesis of alopecia areata and its relation to stem cell involvement is a complicated process. It is known that the damage to stem cells is a result of inflammatory mechanisms; the hair follicle itself is the key target of autoaggresive immunity. In addition, other factors may be involved, including loss of immune protection of stem cells as well as enhanced autoimmunity by environmental factors and genetic predisposition
