Effects of prolonged cold ischemia time on delayed renal graft function

Ciljevi istraživanja: Ispitati povezanost trajanja hladne ishemije i odgođene bubrežne funkcije, ispitati karakteristike ispitanika i funkciju presatka između skupine s odgođenom bubrežnom funkcijom i skupine bez odgođene bubrežne funkcije. Nacrt studije: Retrospektivna kohortna studija Ispitanici i metode: 42 transplantirana pacijenta (52 % muškaraca i 48 % žena) medijana dobi 55 godina. Svi su pacijenti prethodno bili na hemodijalizi. Svi su presatci organi mrtvih donora kojima je dijagnosticirana moždana smrt. Pacijenti su na temelju pojave odgođene bubrežne funkcije podijeljeni u dvije skupine. Rezultati: Trajanje hladne ishemije medijana je 872,5 minute (14,5 sati) (interkvartilnog raspona 11 sati do 17 sati) i nije statistički povezano s odgođenom bubrežnom funkcijom. 24 % ispitanika ima odgođenu bubrežnu funkciju. Između dviju skupina nema razlike među spolovima, starosti niti nepodudarnosti HLA alela. Indeks tjelesne mase značajno je viši u skupini s odgođenom bubrežnom funkcijom. Indeks otpora, vrijednosti kreatinina i klirens kreatinina u prvom su mjerenju bez značajne razlike u objema skupinama ispitanika, a značajno više vrijednosti indeksa otpora i kreatinina, i značajno niže klirensa kreatinina u drugom mjerenju imaju ispitanici s odgođenom bubrežnom funkcijom. Nepodudarnost HLA alela nije povezana s odgođenom bubrežnom funkcijom. Nije utvrđena povezanost nastanka akutne tubularne nekroze i trajanja hladne ishemije. Zaključak: Nije utvrđena povezanost trajanja hladne ishemije i odgođene bubrežne funkcije kao ni povezanost razvoja akutne tubularne nekroze s produljenom hladnom ishemijom. Pacijenti s odgođenom bubrežnom funkcijom imali su značajno lošiju bubrežnu funkciju pred otpust iz bolnice. Rezultati istraživanja u skladu su s većinom referenciranih istraživanja.Objectives: The aim of this study was to determine the correlation between cold ischaemia time and delayed graft function. Furthermore, the aim was to compare patients' characteristics and graft functions between two studied groups, as well as to study the correlation between cold ischaemia time and acute tubular necrosis. Study design: Retrospective cohort study Patients and methods: This study included 42 patients with kidney transplant (52% male, 48% female). Age median was 55 years. All patients were on dialysis prior to the transplantation. All transplants are from deceased donors with diagnosed brain death. Patients were divided into two groups depending on delayed graft function. Results: Median of cold ischaemia time is 872.5 min (14.5 hours). There is no significant statistical correlation between cold ischaemia time and delayed graft function. 24% of patients had delayed graft function. There is no difference in sex, age or HLA mismatch between the two groups. Body mass index is significantly higher in patients with delayed graft function. There is no statistically significant difference between resistance index, creatinine levels and creatinine clearance in the first postoperative medical report. In the second report there are significantly higher creatinine levels, higher resistance index and lower creatinine clearance in patients with delayed graft function. HLA mismatch is not significantly connected with delayed graft function. There is no significant correlation between acute tubular necrosis and cold ischaemia time. Conclusion: Connection between cold ischaemia time and delayed graft function was not determined. The study did not confirm the correlation of cold ischaemia time and acute tubular necrosis. Patients with delayed graft function had significantly worse kidney function when released from the hospital care. Results of this study are in correlation with most referenced works in the field of kidney transplantation
