The Austrian Thrombotic Microangiopathy Registry (ATMAR): A retrospective analysis of underlying conditions and response to treatment in the chronic course of thrombotic thrombocytopenic purpura

Hintergrund: Thrombotische Mikroangiopathien (TMAs) sind eine diverse Gruppe von Krankheiten, die die klinischen Merkmale Thrombozytopenie, mikroangiopathische hämolytische Anämie und Endorganschaden zufolge mikrovaskulärer Thrombose teilen. Erworbene thrombotisch thrombozytopenische Purpura (aTTP) ist eine TMA, die durch autoimmunologische Hemmung des ADAMTS13-Enzyms ausgelöst wird, welches für die Spaltung des von Willebrand Faktors (VWF) verantwortlich ist. Es wurde ein landesweites Register zur Erfassung von TMA-Patienten geschaffen, das Austrian Thrombotic Microangiopathy Registry (ATMAR). Statistische Auswertungen der Daten von aTTP-Patienten wurden in Bezug auf chronischen Krankheitsverlauf untersucht. Methoden: Daten von TMA-Patienten zwischen Jänner 1991 und Dezember 2017 wurden retrospektiv gesammelt und statistisch ausgewertet. Resultate: Es wurden 74 Patienten ins ATMAR aufgenommen, darunter 59 Patienten mit aTTP; 18 Männer (medianes Alter: 48,5 Jahre) und 41 Frauen (medianes Alter: 40,4 Jahre), die zwischen 1 Tag und 23,1 Jahren beobachtet wurden. 84 hämatologische Episoden wurden aufgezeichnet, mit signifikant stärker alterierten Laborwerten in Erstmanifestationen als in Rezidiven. 72,1% der Episoden waren nicht mit auslösenden Konditionen assoziiert; diese beeinflussten nicht die Rückfallrate. Es wurden 87 autoimmunologische Episoden von 55 Patienten aufgezeichnet; 75% davon gingen nicht mit hämatologischen Episoden einher. Die Zeit zur autoimmunologischen Remission war im Median 42 Tage in initialen und 185 Tage in autoimmunologischen Rezidivepisoden (Unterschied signifikant während des ersten Jahres mit p=0,010). Zu 29,5% persistierten anti-ADAMTS13-IgG Antikörper zum Remissionszeitpunkt. Die Zeit (ab Remission) bis zu autoimmunologischen Rückfällen betrug 7,1 Jahre zum ersten, und 2,7 Jahre zu weiteren Rückfällen (p=0,79), und war signifikant länger in den ersten 2 Jahren nach Rituximab Behandlung (p=0,001). 13,6% der Patienten verstarben während der initialen Episode an Myokardinfarkten. Das Gesamtüberleben betrug 86,4% nach 5 Jahren. Interpretation: Die aTTP hat eine gute Prognose, sofern das initiale Ereignis überlebt wird. Autoimmunaktivität im chronischen Krankheitsverlauf ist oft nicht mit hämatologischen Befunden assoziiert. Mehr als ein Drittel der Patienten hatten wiederkehrende autoimmunologische Aktivität.Background: Thrombotic microangiopathies (TMAs) are a diverse group of diseases that share the clinical defining features of thrombocytopenia, microangiopathic hemolytic anemia and end organ damage due to microvascular thrombosis. Acquired thrombotic thrombocytopenic purpura (aTTP) is a TMA that is caused by autoimmunological inhibition of the ADAMTS13 enzyme, which is responsible for cleavage of von Willebrand factor (VWF) to maintain its proper function. A nationwide registry with patients suffering from TMA syndromes was established, the Austrian Thrombotic Microangiopahty Registry (ATMAR), and statistical analysis on patients suffering from aTTP was performed, with a focus on the chronic disease course. Methods: Data on patients suffering from TMAs between January 1991 and December 2017 was collected and analyzed in a retrospective manner. Results: 74 patients were included in the ATMAR. 59 patients suffering from aTTP, 18 male (median age: 48.5 years) and 41 female (median age: 40.4 years), and were followed for 1 day to 23.1 years (median: 4.9 years). 84 hematologic episodes were recorded, with more pronounced laboratory alterations in initial hematologic episodes compared to relapses. 72.1% of these episodes were not associated with underlying conditions, which had no significant influence on the relapse rate. 87 autoimmunological episodes were recorded in 55 patients. 75% of autoimmunological relapses occurred without a concurrent hematologic relapse. Median time to autoimmunological remission was 42 days for initial autoimmunological episodes and 185 days for autoimmunological relapses (significant difference over the first year after onset with p=0.010). In 29.5% anti-ADAMTS13-IgG persisted at the time when ADAMTS13 activity reached >20%. Median time (since remission) to autoimmunological relapse was 7.1 years for first-time, and 2.7 years for further relapses (p=0.79), and was significantly longer in the first 2 years after rituximab treatment, with p=0.001. 13.6% of patients died, all during their initial episode due to myocardial infarction. Overall survival was 86.4% after 5 years. Interpretation: aTTP has good prognosis when the initial episode is survived. Autoimmunological activity in the chronic course of the disease is many times not associated with hematologic findings.
