Analysis of the physical therapeutic approach influence over fatigue in individuals with multiple sclerosis : a systematic review

Monografia (graduação)—Universidade de Brasília, Faculdade de Ceilândia, Curso de Fisioterapia, 2013.A Esclerose Múltipla (EM) é uma doença neurodegenerativa que acomete progressivamente a bainha de mielina, levando a déficits cognitivos e funcionais, sendo a fadiga um sintoma incapacitante relatado por 80% dos indivíduos. O objetivo do estudo foi avaliar a influência da abordagem fisioterapêutica sobre a fadiga relatada por indivíduos com EM. Os artigos utilizados na revisão foram encontrados nas bases de dados SciELO, PEDro, Pubmed, Cochrane, Bireme e Science Direct. As palavras- chaves na primeira busca foram esclerose múltipla (multiple sclerosis), fadiga (fatigue), fisioterapia (physical therapy) e reabilitação (rehabilitation) e na segunda, esclerose múltipla (multiple sclerosis), fadiga (fatigue) e fisioterapia (physical therapy). A busca foi limitada para estudos publicados nos últimos 5 anos e nas línguas portuguesa, inglesa, espanhola e francesa. Foram excluídos estudos de revisão, estudos transversais, outras modalidades de intervenção além da fisioterapia e estudos com outras condições de saúde. Foram encontrados 11 estudos, sendo 4 ensaios clínicos randomizados e 7 diferentes tipos de estudos experimentais. Verificou-se que a abordagem sobre a fadiga baseia-se em fisioterapia convencional, terapia de restrição e indução do movimento, reabilitação vestibular, hidroterapia e eletroestimulação. Apenas 1 estudo diferenciou fadiga central e fadiga periférica. Como sintoma incapacitante, a fadiga da EM deve ser abordada de maneira eficaz. Os estudos atuais abordam fadiga de maneira geral, não diferenciando seu componente central e seu componente periférico. Sugere-se uma abordagem baseada em exercício físico, treino orientado à tarefa específica e eletroestimulação, além de mais estudos na área. __________________________________________________________________________ ABSTRACTMultiple Sclerosis (MS) is a neurodegenerative and progressive disease that affects myelin sheath, leading to cognitive and functional deficits, with fatigue as the main disabling symptom reported by 80% of individuals. The objective was to evaluate the influence of physical therapeutic approach on the symptom of fatigue. The articles on this systematic review were found in the SciELO, PEDro, Pubmed, Cochrane, Bireme and Science Direct database. The used keywords were multiple sclerosis, fatigue, physical therapy and rehabilitation; in the second round, multiple sclerosis, fatigue and physical therapy. The search was constrained by published articles in the last 5 years and by the ones in Portuguese, English, Spanish and French languages. Review and transversal studies were excluded, as well as other types of interventions besides physical therapy, and studies with other health condition. Eleven studies were found including 4 controlled randomized trial and 7 other types. It was seen that the approach to fatigue is based on conventional physical therapy, constraint-induced movement therapy, vestibular rehabilitation, hydrotherapy and electrical stimulation. Only one study differentiated central fatigue to peripheral fatigue. As a disabling symptom in MS, fatigue must be dealt in an effective manner. Current studies focus on fatigue in a general manner not taking into consideration the differences between peripheral and central components. It is suggested an approach based on physical exercise, task- specific training and electrical stimulation, in addition to further studies about this subject
