Heart failure is a major public health problem in the developed countries. In the past 5 years there has been a strategic shift in the therapy of heart failure. In the 1960 to 1995 era, persons with overt clinical heart failure, which was associated with typical symptoms and signs, had been tar-geted. However, recent data suggested that despite of advances in heart failure therapy, improvement of survival for symptomatic patients in the community remained low. Therefore, early detection of ventricular dysfunction and prevention of symptomatic heart failure have been given a high priority.
