Interprofessional collaboration among pharmacists and physicians: together against prescription errors.

Interprofessional collaboration between pharmacists and physicians to conduct medication reviews is important for optimizing the medical treatment of patients suffering from chronic illnesses. The pharmacist as a final gate keeper is the one who is the last to detect errors and prevent patients. The pharmaceutical which is not correctly prescribed and then given to the patient is 100% harmful for patients’ health. According to the literature sources from other countries, we can suspect that the interprofesional cooperation between pharmacists and medical doctors is accepted and has shown positive results so far as applyed in the treatment of lethal diseases
