Seleção in vivo de peptídeos ligantes à articulações inflamadas e seu potencial uso em diagnóstico

CHAPTER I: The immune system is a complex of molecules and cells that functions to protect the organism against pathogens. The healthy immune system must maintain the balance between the ability to respond to infectious agents and sustain self-tolerance. However, failures may occur which result in reactions against cells and tissues of the organism and which cause autoimmune diseases such as Rheumatoid Arthritis (RA). This is an autoimmune disease characterized by symmetrical arthritis and additive, of unknown cause, usually beginning after age 30 and affects three times more women than men, with a prevalence of 1.0% in some regions. In rheumatoid arthritis, the synovial membrane becomes infiltrated by various types of inflammatory cells, which synergize to cause destruction of joints, resulting in strong joint pain. The diagnosis of arthritis depends on the association of a number of clinical signs and symptoms, laboratory findings, and radiographic findings. The goal of treatment is to relieve pain, maintain or improve functional capacity, prevent disability and improve their quality of life. Although there is no test that confirms the presence of the disease, there is no specific laboratory tests considered for confirming diagnosis. Murine models are effective to enrich our understanding of human arthritis, as DBA/1J strain animals presenting high susceptibility to development of arthritis resulting from immunization with type II collagen. Phage display is a selection method wherein a library of peptides that are expressed on the surface of phage, with genetic material coding for each peptide located in the viral genome. The targeting peptide ligands the highly inflamed regions are useful to assist in differential diagnosis, assess the extent of inflammation and determining prognosis and response to therapy. Accordingly, the search for novel biomarkers may lead to new methods for the diagnosis and treatment of RA. This review aims to describe some aspects of Rheumatoid Arthritis and how technology Phage Display in vivo can be used to identify ligands to various antigens associated with inflammation. CHAPTER II: Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by joints persistent inflammation. Pain, swelling and limitation of movement are the typical signs of this inflammation. Arthritis is the name given to a joint inflammation, with a barely known root, and may affect different joints. The diagnosis of RA depends on the association of a number of symptoms and clinical signs, laboratory and radiographic examinations. Thus, studies directed to investigate new biomarkers become of great importance for the diagnosis of RA. The aim of this study was to select and identify the methodology by Phage Display in vivo binder peptides to inflamed regions of mice DBA / 1J with high inflammation induced by collagen type II. For induction of arthritis, mice of isogenic DBA/1J were immunized with 100ug of type II collagen emulsified with complete Freund\'s adjuvant. For the selection of binder peptides to the inflamed regions was performed an in vivo bioppaning using a peptide library Ph.D.-C7C expressed on the surface of filamentous phage M13 for three cycles of selection, the last one being performed with subtraction of nonspecific using healthy joints. The DNA of selected clones was sequenced, translated and several clones were subjected to the tests Immunohistochemistry. Among the 144 clones sequenced, 115 had valid sequences. Among the last ones were identified 20 different sequences. The bioinformatics analysis demonstrated that there is no consensus among the majority of selected peptides. O sistema imune sadio deve manter o balanço entre a capacidade de responder a agentes infecciosos e de sustentar a autotolerância. No entanto, podem ocorrer falhas que resultam em reações contra as células e tecidos do próprio organismo e que causam doenças autoimunes, como a Artrite Reumatóide (AR). Esta é uma doença autoimune caracterizada por artrite simétrica e aditiva, de causa desconhecida, que se inicia geralmente após os 30 anos, acometendo mulheres três vezes mais que em homens, com prevalência de 1,0% em algumas regiões. Nas artrites reumatóides, a membrana sinovial torna-se infiltrada por vários tipos de células inflamatórias, que sinergizam para causar destruição das articulações, provocando fortes dores articulares. O diagnóstico da artrite depende da associação de uma série de sintomas e sinais clínicos, achados laboratoriais e radiográficos. O objetivo do tratamento é aliviar as dores, melhorar ou manter a capacidade funcional, prevenir as incapacidades e melhorar sua qualidade de vida. Ainda não existe nenhum teste que confirme a presença da doença, não há exames laboratoriais considerados específicos para a sua definição diagnóstica. Modelos murinos são eficazes para ampliar nossa compreensão a respeito da artrite humana, como animais da linhagem DBA/1J que apresenta alta susceptibilidade ao desenvolvimento de artrite consequente de imunização com colágeno tipo II. Phage Display é um método de seleção no qual uma biblioteca contendo peptídeos que são expressos na superfície de fagos, com o material genético codificante para cada peptídeo localizado no genoma viral. O direcionamento com peptídeos ligantes à regiões altamente inflamadas são úteis para auxiliar no diagnóstico diferencial, avaliar a extensão da inflamação, determinar o prognóstico e resposta à terapia. Nesse sentido, a busca por novos biomarcadores podem levar a novos métodos para o diagnóstico e tratamento da AR. Essa revisão visa descrever alguns aspectos da Artrite Reumatóide e como a tecnologia de Phage Display in vivo pode ser utilizada na identificação de ligantes à diversos antígenos relacionados à inflamação. CAPÍTULO II: A Artrite Reumatóide (AR) é uma doença autoimune crônica, caracterizadas por inflamação persistente das articulações, sendo os sinais típicos desta inflamação a dor, inchaço e limitação dos movimentos. O termo Artrite se dá a uma inflamação da articulação, de causa pouco conhecida, podendo acometer diferentes articulações. O diagnóstico da AR depende da associação de uma série de sintomas e sinais clínicos, achados laboratoriais e radiográficos. Desta forma, trabalhos que são direcionados a investigar novos biomarcadores tornam-se de grande importância para o diagnóstico da AR. O objetivo deste estudo foi selecionar e identificar por meio da metodologia de Phage Display in vivo, peptídeos ligantes à regiões inflamadas de camundongos DBA/1 sintomatizados com alta inflamação induzida por colágeno tipo II. Para indução da artrite, camundongos de linhagem isogênica DBA/1J foram imunizados com 100ug de colágeno tipo II emulsificado com adjuvante de Freund completo. Para seleção de peptídeos ligantes à regiões inflamadas foi realizado um bioppaning in vivo utilizando uma biblioteca de peptídeos Ph.D.-C7C expressa na superfície do fago filamentoso M13 por três ciclos de seleção, sendo a última realizada com subtração dos inespecíficos utilizando articulações sadias. O DNA dos clones selecionados foi sequenciado, traduzido e os diversos clones foram submetidos aos ensaios de Imunohistoquimica. Dentre os 144 clones sequênciados, 115 apresentaram sequências válidas, dos quais foram identificadas 20 sequências diferentes. As análises de bioinformática demonstraram que não há consenso entre a maioria dos peptídeos selecionados. O ensaio de Imunohistoquimica permitiu a pré-validação de dois clones com potencial biomarcador para o imunodiagnóstico da Artrite Reumatóide
