Epidemiology of notified cases of human and bovine tuberculosis in a dairy production region

Tuberculosis is a disease caused by different agents of the complex Mycobaceterium tuberculosis, with a complex epidemiology involving different hosts, presence of comorbidities and other risk factors. One Health approach could aid the TB control process in humans, especially dairy cattle producer regions, because raw milk and dairy products is the main source of infection in bovine-human transmission. The objective of this study was to analyze risk factors for human TB and its relationship with cases of bovine tuberculosis in a region with higher dairy production in Brazil. Cross-sectional study involving all cases of TB reported to the Brazilian Notifiable Diseases Surveillance System (Sistema de Informações de Agravo de Notificação - SINAN) in Brazil between 2011 and 2015. Sociodemographic and clinical characteristics of patients with exclusively PTB and exclusively EPTB were compared. Following analysis with Pearson’s chi-square test, variables with p < 0.05 were included in a hierarchical regression model. Variables with p < 0.05 in the corresponding level were kept in the model. It was selected 1,483 cases of tuberculosis in humans were selected in the areas analyzed, being 314 extrapulmonary TB and 1189 TB of the pulmonary. By means of hierarchical regression analysis, it is possible to show that the low level of schooling for the pulmonary form is a risk factor for extrapulmonary HIV infection was prominent. Although tuberculosis is present with moderate prevalence in the properties of the studied region, the differential diagnosis for M. bovis in cases of human tuberculosis is not performed, resulting in underreporting or negligence with TB by M. bovis. Uma abordagem em Saúde Única pode auxiliar o processo de controle da TB em humanos, especialmente regiões de produtoras de leite bovino, haja vista que a ingestão de leite cru e derivados é a principal fonte de infecção na transmissão bovino-humano. Objetivou-se, com o presente estudo analisar fatores de risco para TB humana e suas relações com os casos de tuberculose bovina em região de maior produção leiteria do Brasil. A partir dos dados de notificação de TB humana no Sistema Informação de Agravos de Notificação, no período de 2011 a 2015, foram selecionados 1.483 casos de tuberculose em humanos nas áreas analisadas, sendo 314 TB extrapulmonar e 1189 TB pulmonar. Por meio da análise de regressão logística multivariada hierarquizada foi possível evidenciar como fatores de risco a baixa escolaridade para forma pulmonar e ser portador do HIV para forma extrapulmonar. As informações sobre as notificações de bovinos reagentes positivos as provas intradérmicas oficiais foram obtidos no Instituto Mineiro de Agropecuária, responsável por receber os relatórios de notificação, para o período de 2011 a 2017. Para análise de condenação de carcaças por suspeita de tuberculose foram avaliados o abate diário de três frigoríficos na área de estudo, onde foi analisado idade do animal e destinação da carcaça após a condenação. Através de regressão linear observou-se tendência decrescente nas notificações de reagentes positivos e inconclusivo. Já os dados de condenação de carcaça demonstraram-se estacionários, com maior número de fêmeas condenadas, e lesões características de processo de generalização da doença. A discordância entre os dados de notificação e abate, e ausência de animais abatidos acompanhados de atestado positivo declarando o status sanitário do animal, expõe ao risco todas as pessoas envolvidas no manejo, transporte e no abate desses bovinos. Ainda que a tuberculose esteja presente com prevalência moderada nas propriedades da região estudada, o diagnóstico diferencial para M. bovis em casos de tuberculose humana não é realizado resultando em subnotificações ou negligência com TB por M. bovis. A obrigatoriedade de comunicação entre os serviços de defesa animal e saúde pública não vem sendo cumprida, os presentes resultados trazem alerta para falta de conectividade entre as áreas de vigilância da TB o que exige do profissional de Medicina Veterinária uma abordagem em Saúde Única com educação em saúde, acompanhamento do abate sanitário ou eutanásia do bovino reagente positivo, orientação dos serviços de saúde humana para investigação de casos suspeitos, proteção à saúde dos ecossistemas
