Relatives of patients with a diagnosis of Brain Death and the relationship with the patient care team in an Intensive Care Unit

Brain death represents the irreversible clinical state in which brain functions (telencephalon and diencephalon) and brainstem are irremediably impaired. Although often avoided in our society, the theme of death and dying should be discussed multiprofessionally, aiming at an integrate and humanized attention to the patient and family. The research aimed to investigate, retrospectively, the meanings attributed by family members on the relationship established with the professionals of the multiprofessional health team in the process of verification and diagnosis of brain death. The sample consisted of relatives of 13 patients who were diagnosed during the study period and admitted to the Adult ICU, with semi-structured interviews and self-administered questionnaires. When submitted to inclusion and exclusion criteria, the study was performed with relatives of two patients included. After conducting the interviews and transcriptions in full, the Discourse Analysis methodology was used to categorize the thematic axes and qualitative data analysis, being divided from discursive cuts in "before", "during" and "after", using as reference the hospitalization of the patient. The results aim to propose improvements and advances in the multiprofessional performance and established relationship with the relatives in front of the diagnosis of Brain Death. According to the literature consulted, it was based on the importance of the relationship established with the multiprofessional team, in the way in which the team-family bonding and the communication contribute with the coping, with the understanding of the diagnosis, the decision process on organ donation and mourning the relatives.A morte encefálica representa o estado clínico irreversível em que as funções cerebrais (telencéfalo e diencéfalo) e do tronco encefálico estão irremediavelmente comprometidas. Embora muitas vezes seja evitado em nossa sociedade, o tema da morte e do morrer deve ser discutido multiprofissionalmente, visando uma atenção integral e humanizada ao paciente e familiares. A presente pesquisa buscou investigar, de forma retrospectiva, os sentidos atribuídos por familiares sobre a relação estabelecida com os profissionais da equipe multiprofissional de saúde no processo de verificação e diagnóstico de morte encefálica. A amostra foi composta a partir de familiares de 13 pacientes que obtiveram o diagnóstico no período pesquisado e internados na UTI Adulto, sendo realizadas entrevistas semiestruturadas e questionários autoaplicados. Ao serem submetidos à critérios de inclusão e exclusão, a presente pesquisa foi realizada com familiares de dois pacientes incluídos. Após realização das entrevistas e transcrições na íntegra, foi utilizada a metodologia de Análise do Discurso para categorização de eixos temáticos e análise dos dados, sendo divididos a partir de recortes discursivos em “antes”, “durante” e “depois”, tendo como referência a internação do familiar. Os resultados podem contribuir na compreensão e avanços da atuação multiprofissional e relação estabelecida com os familiares frente ao diagnóstico de Morte Encefálica. Conforme demonstra a literatura consultada, a presente pesquisa demonstrou a importância da relação estabelecida com a equipe multiprofissional, na forma pela qual o vínculo equipe-família e a comunicação contribuem com o enfrentamento, com a compreensão do diagnóstico, o processo de decisão sobre doação de órgãos e luto dos familiares
