The present article reflects empirical observations of professional experience of a Social Worker in an interdisciplinary team: “Care of Patients in Critical Condition” of the “Program of Multi professional Residency in Health” of the Federal University of Uberlândia. It aims to identify the importance of Social Work in the process of patient discharge following hospitalization. It is recommended that action based on the principles that guide the Unified Health System - SUS, which give priority to an expanded concept of health of the Health Reform movement rather than the biomedical care concept.
