Die NOS produziert unter bestimmten Bedingungen O2- anstelle von NO. Dieses Phänomen wird als Entkopplung bezeichnet und tritt auf, wenn das Substrat Arginin und/oder der Cofaktor BH4 fehlt. Mögliche Quellen von O2 sind die Flavine in der Reduktase-Domäne und das Häm in der Oxygenase-Domäne. BH4 kann eine entkoppelte Reaktion entweder durch Dimerstabilisierung oder durch Reduktion des Oxyferro-Komplexes verhindern. Im ersten Fall ist das Enzym durch Monomerisierung nicht in der Lage, Elektronen auf die andere Untereinheit zu übertragen, stattdessen reduzieren Flavine O2 zu O2-.
