Uveítis y enfermedad sistémica: aproximación clínico-diagnóstica enurgencia hospitalaria

Purpose: The relationship between uveitis and systemic disease at Emergency Department (ED) remains unknown. From a multidisciplinary approach, we study its clinical features. Methods: Patients attended at ED of Hospital Universitario de Canarias, Tenerife, Spain, from January 1996 to December 1997, with initial diagnostic of uveitis. We analysed retrospectively their epidemiological and clinical features, and the relationship of initial diagnosis at ED, with final diagnostic at the Rheumatology and Ophthalmology Departments. Statistical analysis was performed by SPSS 7.5 for windows program. Results: During the study period 71patients were included ( 0.048 % of visits to ED ). Sixty four point eight percent were female ( mean age 45.89 years ), and male presents 36.79 years as mean age. There weren't signifficative difference. Seventeen point one percent patients were diagnosticated as Ophthalmologic Specific Syndromes, 35.2% as Idiopathic uveitis, 28.3% as Anterior Uveitis associated with HLA B27 and 19.4% as Secondary Uveitis. Only 10.52 percent patients were studied by Rheymatologist, despite HLA study were performed in 29.9 percent of patients, with positive results in 64.7 percent or them. Conclusions: In most cases, a clinical history, physical exam and basic blood analysis it's enough in the first diagnosis of uveitis. Due to the high relation between uveitis and systemic disease, multidisciplinary study protocols are necessary for the final diagnostic.Objetivo: En la urgencia hospitalaria no es bien conocida la relación entre uveítis y enfermedad sistémica. Presentamos sus características clínico-diagnósticas desde un enfoque multidisciplinar. Material y Método: De los pacientes atendidos en el Servicio de Urgencias del Hospital Universitario de Canarias durante dos años (Enero 1996-Diciembre 1997), se seleccionaron aquellos con diagnóstico inicial de uveítis. Retrospectivamente se analizaron sus características epidemiológicas y clínicas, comparando el diagnóstico inicial de Urgencias con el enfoque diagnóstico final tras la revisión por los servicios de Oftalmología y Reumatología. Para el análisis estadístido se utilizó el programa SPSS 7.5. Resultados: Presentaron uveítis 71pacientes (el 0.048% del total de 147.916 urgencias del período de estudio). El 64.8% fueron mujeres, con una edad media de 45.89 años, mientras que en los varones fue de 36.79 años, no habiendo diferencia estadísticamente significativa. Se diagnosticaron el 17.1% como Síndromes Oftalmológicos específicos, 35.2% de Uveítis Idiopáticas, 28.3% de Uveítis anterior asociada al HLA B27 y 19.4% de Uveítis Secundarias. Sólo el 10.52 % fueron valorados por Reumatología, aunque se realizó estudio HLA al 29.9% de los pacientes, siendo positivo en el 64.7% de éstos. Conclusiones: La anamnesis, la exploración física y una determinación analítica básica son suficientes para orientar el diagnóstico en la mayoría de los pacientes con uveítis. Sin embargo, dada la alta relación entre uveítis y enfermedad sistémica, deben establecerse protocolos multidisciplinares para alcanzar un diagnóstico definitivo
