Baker's Asthma: physiopathogenic particularities

Trabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaBakers’ asthma is considered one of the most common types of work-related asthma, affecting 4 to 25% of those working in panification industry, being classified as occupational when it is caused by allergens present at the work environment or when it gets substancially worse by the professional practice if there was already a previous asthma diagnosis.Different types of flours are the main allergic sources, espefically those of the wheat, rye, barley, oat and rice. Legume-based flours (for exemple, made with soybeans or lupini beans), constitute a whole new allergen source in the baking industry. Other important allergic sources include the aditives (α-amylase, cellulase, protéase, xylanase and glucoamylase); ingredients like eggs, milk, buckwheat, sesame seeds and nuts and dried fruit, and the ocasional flour infestants (mites, fungi and bugs).The main allergens present in the panification business are, undoubtedly, those present in flours (familly of α-amylase/trypsin inhibitors, lipid transfer protein (LTP), peroxidase, thioredoxin, serine protease inhibitors, thaumatin-like protein and some prolamins, mainly gliadins and glutenins) and the fungal enzyme α-amylase, used as a flavour enhancer. It is important to mention that the main allergens responsible for the devolopment of this medical condition is part of the familly of α-amylase/trypsin inhibitors, which are also responsible for the cross reaction between flours of different cereals. Tri a 14 has allso been recognised as an important allergen associated with Baker’s asthma.Concerning the immunological mechanisms involved, we have to consider firstly the IgE mediated reaction, which constitutes the main pathogenic event and determines a classic type I hypersensitivy response, that includes Th2 expressing IL4, IL5 and eosinophilic infiltration. Other immunological mechanisms (not IgE-mediated) can be involved and they include the production of γ-IFN by CD8 cels, and MIP-2 due the stimulation by endotoxins present at the work environment. Non-immunological mechanims result from the exposition to elevated concentrations of irritative agents,which cause an unspecific inflammation.The previously described etiopathogenic mechanisms culminate in physiopathological events at bronchial level, which are characterized by dyspnea, wheezing, cough and a feeling of thoracic heaviness, in the work environment, induced by the reexposition to the etiological allergens. The involvement of the nasal mucosa, that can preceed the asthma, is defined as occupational rhinitis and includes symptoms such as sneezes, aqueous rhinorrhea, nasopharingeal pruritus and nasal obstruction. Besides rhinitis, conjunctivitis is another comorbidity frequently associated. Sharing the same etiopathogenic mechanisms, they are reported in 49-90% of the workers with Baker’s asthma. Although the allergic contact dermatitis caused by flours is a very frequent comorbidity, it implies another kind of pathogenic mechanism, which determines a type IV hipersensitivity reaction or a type of contact dermatitis caused by proteins.This review pretends to list the classical etiopathogenic events and discuss some important aspects only recently identified, critical to the scientific breakthroughs in this model of occupational asthma.A asma dos padeiros constitui uma das formas mais comuns de asma relacionada com a profissão, afetando 4 a 25% dos trabalhadores da indústria da panificação, classificando-se como ocupacional quando causada por alergénios do ambiente laboral ou agravada pela profissão se existe asma prévia. As farinhas são as principais fontes alergénicas, designadamente as de trigo, centeio, cevada, milho, a aveia e o arroz. As farinhas de leguminosas (soja e tremoço) constituem novas fontes alergénicas na indústria da panificação. Outras importantes fontes alergénicas são os aditivos (α-amilase, celulase, protease, xilanase e glucoamilase); ingredientes como o ovo, o leite, o trigo sarraceno, sementes de sésamo e frutos secos, e ainda infestantes das farinhas (ácaros, fungos e insectos). Os principais alergénios na panificação são os alergénios das farinhas (família dos inibidores da α-amilase/tripsina, lipid transfer protein (LTP), peroxidase, tioredoxina, inibidores da serina protease, proteína thaumatin-like e algumas prolaminas, nomeadamente gliadinas e gluteninas) e a enzima fúngica α-amilase, utilizada como melhorante. Salienta-se que a família dos inibidores da α-amilase/tripsina são os principais alergénios indutores desta doença, e são responsáveis pela reactividade cruzada entre as farinhas de diferentes cereais. O Tri a 14 é também reconhecido como alergénio relevante.Relativamente aos mecanismos imunológicos envolvidos salienta-se a reação mediada por IgE, constituindo o seu principal evento patogénico e determinando uma resposta de hipersensibilidade tipo I clássica, que inclui Th2 com expressão IL4, IL5 e infiltração eosinofílica. Outros mecanismos imunológicos (não mediados por IgE) podem estar envolvidos e incluem a produção de IFN-γ por células CD8, e MIP-2 estimulados por endotoxinas presentes no ambiente laboral. Mecanismos não imunológicos resultam da exposição a elevadas concentrações de agentes irritativos conduzindo a uma inflamação inespecífica. Os mecanismos etiopatogénicos descritos culminam em eventos fisiopatológicos a nível brônquico, caracterizados por dispneia, sibilância, tosse e opressão torácica, em ambiente ocupacional, induzido pela reexposição aos alergénios etiológicos. O envolvimento da mucosa nasal pode preceder a asma, designando-se por rinite ocupacional caracterizada por esternutos, rinorreia aquosa, prurido nasofaríngeo e obstrução nasal. Além da rinite, a conjuntivite é outra co-morbilidade frequentemente associada. Partilhando os mecanismos etiopatogénicos, são reportadas em 49-90% dos trabalhadores com asma dos padeiros. A dermatite de contacto alérgica a farinhas, apesar de ser uma co-morbilidade muito frequente, implica outro mecanismo patogénico, determinando uma hipersensibilidade tipo IV ou uma dermatite de contacto por proteínas.Pretende-se nesta revisão elencar os eventos etiopatogénicos clássicos e discutir os aspetos recentemente identificados, críticos no avanço científico neste modelo de asma ocupacional
