Skin rejection in vascularized composite allotransplantation : novel insights into its mechanism and potential targeted treatment

Die vaskularisierte Gewebstransplantation bietet die einzigartige Möglichkeit, große Gewebsde-fekte und amputierte Gliedmaßen mithilfe eines allogenen Spendertransplantates zu rekonstruieren. In den letzten 15 Jahren konnten über 100 solcher Transplantationen, am öftesten Hand- und Ge-sichtstransplantationen, durchgeführt werden. Die häufigste Komplikation sind Abstoßungsreaktio-nen, welche hauptsächlich die Haut eines solchen Transplantates betreffen und meist T Lymphozy-ten-mediiert sind. Die zugrunde liegenden Mechanismen sind jedoch noch weitgehend ungeklärt. In den hier präsentierten Studien wurden der Phänotyp des dermalen zellulären Infiltrates, Marker der zellulären und humoralen Abstoßung, die Expression von Adhäsionsmolekülen und Toleranzmarker sowie das Phänomen der Neubildung von ektopen Lymphfollikeln im Bereich der humanen und ex-perimentellen vaskularisierten Gewebstransplantation untersucht. Weiters evaluierten wir den Ef-fekt eines Kv1.3 Kaliumkanalblockers, welcher die Aktivierung und Funktion von effektor memory T Zellen hemmt, in einem experimentellen Beintransplantationsmodell in der Ratte. In Hautbiopsien von Handtransplantaten konnten wir zeigen, dass während milden Absto-ßungsreaktionen T Lymphozyten das dermale Zellinfiltrat dominieren und sämtliche Adhäsionsmo-leküle verstärkt in der Haut exprimiert werden. Eine verstärkte Expression von IDO und Foxp3 konn-te vor allem während starken Abstoßungsreaktionen zu einem späten Zeitpunkt nach Transplantati-on festgestellt werden, was aufgrund der tolerogenen Eigenschaften dieser Marker auf eine versuch-te Selbstlimitation des Prozesses schließen lässt. Weiters konnte das Vorhandensein von tertiär lymphatischen Organen (TLO), welche in die chronische Immunstimulation involviert sind, in der humanen, Primaten- und Rattenhaut während Abstoßungsreaktionen nachgewiesen werden. Die Expression von peripheral node addressin in den hohen Endothelien der Venolen der TLO korrelierte mit der Hautabstoßung und dem Vorhandensein von T und B Lymphozyten. Die systemische Verab-reichung des Kv1.3 Kaliumkanalblockers Correolide C zeigte eine signifikante Verlängerung des Transplantatüberlebens im Beintransplantationsmodell in der Ratte. Die Resultate dieser Studien geben einen weiteren, wichtigen Einblick in den Mechanismus der Abstoßung von vaskularisierten Gewebstransplantaten. Ein tieferes Verständnis dieses Prozesses ist unerlässlich in der Entwicklung von neuen, zielgerichteten, lokal in die Haut applizierten Therapien im Bereich der rekonstruktiven Transplantation.Vascularized composite allotransplantation (VCA) is a young transplant discipline offering the unique possibility to reconstruct large tissue defects and missing limbs with allogeneic tissue from a brain dead organ donor. More than 100 such transplants, most of them being hand and face trans-plants, having been performed over the last 15 years. The complication most often observed in these patients is allograft rejection, which primarily affects the skin compartment of a VCA. First insights revealed that this process is mainly a T lymphocyte driven response towards the epidermis. How-ever, the underlying mechanisms and pathophysiology is poorly understood so far. We herein inves-tigated the phenotype of the dermal immune cell infiltrate as well as markers for cellular and anti-body mediated rejection, adhesion molecules, tolerance markers and the phenomenon of lymphoid neogenesis in clinical and experimental VCA. Finally, we evaluated the effect of an inhibitor for the Kv1.3 potassium channel, which is critically involved in the activation and function of effector mem-ory T cells, in an experimental VCA model. In depth investigation of mild (grade I) rejection in skin of human hand allografts revealed a T lymphocyte dominated dermal cell infiltrate and upregulation of adhesion molecules, whereas signs for humoral rejection were sparse. An increased expression of IDO and Foxp3, especially during late, severe rejection episodes, indicates tolerogenic properties of a proportion of the infiltrating cells and thus a tendency towards self-limitation of the alloimmune response. Furthermore, lymphoid neogenesis and the formation of tertiary lymphoid organs (TLO), which have been shown to be in-volved in chronic immune stimulation, were uniformly found in skin of human, nonhuman primate and rat VCA during rejection. Expression of peripheral node addressin in high endothelial venules of TLO correlated with skin rejection and T and B lymphocyte infiltration. Finally, the Kv1.3 potassium channel blocker correolide C demonstrated significant prolongation of allograft survival when ad-ministered systemically in a rat hind limb transplant model. In conclusion, the results of these studies give valuable and useful insights into the mechanism of skin rejection in VCA, which is essential in the development targeted, localized treatment regimens to reduce the risk of conventional, highly toxic immunosuppression.Theresa HautzEnth. u.a. 5 Veröff. d.
