Searching for genetic and non-genetic factors affecting lipoprotein(a) and apolipoprotein A-IV using genome-wide association and Mendelian randomization approaches

Ein Schwerpunkt genetisch-epidemiologischer Forschung liegt auf der Identifikation von genetischen Markern, die für die Ausprägung eines bestimmten Merkmals im Menschen von Bedeutung sind. Seit 2005 werden genomweite Assoziationsstudien vermehrt eingesetzt. Mit deren Hilfe kann untersucht werden, ob Variationen in einer einzelnen Base der DNS einen relevanten Effekt auf einen bestimmten Phänotyp haben. Auch wenn mittlerweile viele Zusammenhänge entdeckt werden konnten, ist das Wissen über das Zusammenspiel genetischer Faktoren mit insbesondere komplexen Phänotypen doch noch sehr begrenzt. Zwei solche Phänotypen sind Lipoprotein(a) [Lp(a)], ein Lipoprotein welches im menschlichen Blut zu finden ist und Cholesterin, Fette und Proteine transportiert und Apolipoprotein A-IV [apoA-IV], ein anti-oxidatives Glykoprotein welches Bestandteil des Cholesterinrücktransportes ist. Das Ziel dieser Dissertation ist die Identifikation genetischer Faktoren, welche einen Einfluss auf die Lp(a)- und apoA-IV-Konzentrationen im menschlichen Körper haben. Drei in Fachzeitschriften veröffentlichte Publikationen bilden das Gerüst dieser Arbeit. Die ersten beiden verwenden genomweite Assoziationsanalysen, um genetische Varianten zu finden, welche mit Lp(a)- (Publikation 1) bzw. mit apoA-IV-Konzentrationen (Publikation 2) assoziiert sind. Da die Lp(a)-Konzentrationen sehr stark durch eine „copy-number-variation“ in der DNS beeinflusst werden, haben wir zusätzlich eine Adjustierung darauf vorgenommen. Die dritte Publikation verwendet das Verfahren der Mendelschen Randomisierung, welche genetische Varianten einsetzt, um, in diesem Fall, den kausalen Zusammenhang von apoA-IV mit nicht-genetischen Faktoren zu untersuchen. Unter anderem konnten wir das APOE2-Allel sowie viele genetische Varianten in der LPA-Genregion identifizieren, welche Assoziationen mit Lp(a) aufwiesen. Außerdem fanden wir SNPs in den APOA4- und KLKB1-Genregionen, die mit apoA-IV assoziiert sind. Die Mendelsche Randomisierung zeigte kausale Einflüsse von Nierenfunktion und Triglyzerid-Konzentrationen auf apoA-IV. Auch wenn noch viele regulatorische Mechanismen bezüglich Lp(a) und apoA-IV ungeklärt bleiben, konnten meine Kollegen und ich einen entscheidenden Beitrag zu deren Aufklärung liefern.The field of genetic epidemiology focuses on the identification of genetic factors and their interaction with environmental factors relevant for the manifestation of a certain trait in human populations. Since 2005, genome-wide association studies [GWAS] are increasingly used in which single-nucleotide polymorphisms [SNPs], i.e. variations in a single base at a specific position of the DNA, are tested for their association with a certain trait. Although a lot of SNP-trait associations have been revealed meanwhile, for many traits, especially for the more complex phenotypes, the knowledge on the genetic regulation and function is still limited. Among those complex phenotypes are lipoprotein(a) [Lp(a)], a lipoprotein that occurs in the human blood carrying cholesterol, fats and proteins and apolipoprotein A-IV [apoA-IV], an antioxidative glycoprotein that participates in reverse cholesterol transport. This PhD thesis addresses the investigation of the genetic basis of Lp(a) and apoA-IV on a genome-wide scale to increase knowledge on their genetic regulation and function. Three peer-reviewed papers constitute the body of this thesis. The first two papers comprise genome-wide meta-analyses with respect to the traits Lp(a) and apoA-IV with the aim to find new genetic variants that are associated with the concentration of either Lp(a) or apoA-IV. As Lp(a) concentrations are highly dependent on a copy-number-variation within the LPA gene, we included an adjustment for it to find genetic variants that are independent thereof. The third paper deals with the analysis of possible causal relations of apoA-IV with disease-related traits that were found to be associated with apoA-IV by the application of Mendelian randomization. As main results we identified a large number of independent SNPs in the LPA gene region as well as the APOE2 allele to be significantly associated with Lp(a) concentrations. For apoA-IV, we identified the two genomic regions around the APOA4 gene and the KLKB1 gene to be associated with apoA-IV concentrations. Using the Mendelian randomization approach we found causal associations of kidney function and triglycerides on apoA-IV. Although there is still a lot to discover, the projects conducted by me and my colleagues added important new insights into the regulation of Lp(a) and apoA-IV.Salome R.
