Bilan d'activité opératoire (janvier-mai 2017) du service de Dermatologie de l'Hôpital Nord du CHU de Marseille

Introduction: Due to the rise in number of skin cancers, the surgical activity of dermatologists increased significantly in the last few years.The main goal of this study was to analyze in a forecasting manner over 5 months the activity of the surgical Dermatology department of the University Hospital “Centre Hospitalier et Universitaire Nord de Marseille”.The secondary goal was to study the impact of potential post-operative complications in order to evaluate the safety of exeresis procedures in a dermatologic ambulatory setting and to identify potential complicating factors.Material and methods: Every patient on whom an exeresis procedure was performed between January 1st, 2017 and May 31st, 2017 were included. A questionnaire gathering their main demographic data and medical information was systematically filled out before the intervention, as well as a post-operative follow-up form. The statistical analysis was accomplished using the software PASW statistics edition 17.0.2 (SPSS Inc, Chicago, IL, USA).Results: 868 exeresis procedures were performed during the study period on 636 patients. The sex-ratio was close to 1 and the average age was 56.9 (standard deviation 18.7). The average number of comorbidities per patient was 1.92 with a standard deviation of 1.75. The time before receiving treatment was relatively long with an average wait of 32.6 days (standard deviation 28.7). The majority of our activity (61.8%) was regarding lesions suspicious of malignancy at the diagnosis. The lesions were histologically documented before exeresis only in 10.2% of cases. The diagnosis predictability was very satisfying with a consistency between the diagnosis invoked and the anatomopatholigic result in 82.2% of cases. The exeresis rate in sound margins was also excellent with an average rate of 95.3% and the exeresis margins were adapted to the anatomopathologic subtype in 87.8% of the cases. The overall complication rate was low by about 2.3%.Conclusion: This study confirms the important activity of the Surgical Dermatology department within a service of a French University Hospital in 2017. Due to their good recognition of skin lesions and of their limits, dermatologists offer an optimal care with low morbidity to patients requiring skin surgery. The surgical aspect of dermatology deserves to be further developed and organized in the years to com.Introduction : du fait de l’augmentation de l’incidence des cancers cutanés, l’activité chirurgicale des dermatologues a augmenté de manière significative ces dernières années.L’objectif principal de notre étude était d’analyser de manière prospective sur 5 mois l’activité du département de Dermatologie chirurgicale du Centre Hospitalier et Universitaire Nord de Marseille. L’objectif secondaire était d’étudier l’incidence d’éventuelles complications post opératoires afin d’évaluer la sureté des gestes d’exérèse en milieu dermatologique ambulatoire et d’identifier d’éventuels facteurs de complications.Matériels et Méthodes : tous les patients pour lesquels un acte d’exérèse était effectué entre le 1/01/2017 et le 31/05/2017 étaient inclus. Un questionnaire recueillant leurs principales données démographiques et informations médicales était systématiquement rempli avant l’intervention ainsi qu’une fiche de suivi de complication post opératoire. L’analyse statistique a été réalisée à l’aide du logiciel PASW statistics version 17.0.2 (SPSS Inc, Chicago, IL, USA).Résultats : 868 actes d’exérèse ont été réalisés sur la période d’étude chez 636 patients. Le sex-ratio était proche de 1 et l’âge moyen était de 56,9 ans (écart-type 18,7). Le nombre de comorbidités moyen par patient était de 1,92 avec un écart-type de 1,75. Le délai de prise en charge était relativement long avec un délai moyen de 32,6 jours (écart-type 28,7). La majorité de notre activité (61,8%) concernait des lésions suspectes de malignité au diagnostic. Les lésions n’étaient documentées histologiquement avant exérèse que dans 10,2% des cas. La prédictibilité diagnostique était très satisfaisante avec une concordance entre le diagnostic évoqué et le résultat anatomopathologique dans 82,2% des cas. Le taux d’exérèse en marges saines était également excellent avec un taux moyen de 95,3% et les marges d’exérèse étaient adaptées au sous-type anatomopathologique dans 87,8% des cas. Le taux de complication globale était faible d’environ 2,3%.Conclusion : cette étude confirme l’activité importante du département de Dermatologie Chirurgicale au sein d’un service de CHU Français en 2017. De par sa bonne reconnaissance des lésions cutanées et de leurs limites le dermatologue offre une prise en charge optimale avec une faible morbidité aux patients nécessitant une chirurgie cutanée. Le versant chirurgical de la dermatologie mérite d’être encore développé et organisé dans les années à venir
