Guía asistencial para el manejo de las pacientes con miomas uterinos

Curs acadèmic: 2017/2018Introduction: The uterine myomas are benign gynecological tumors with a prevalence of 70%, even more in the black race and with a family history. They are a common cause of abnormal menstrual bleeding and pressure - derived symptoms. Their high prevalence makes them cons idered a major problem, affecting the quality of life of patients and causing a significant social and economic impact. Objectives: To realize a guide of clinical practice that orients in the decision making for the diagnosis and treatment of the patients with suspicion or diagnosis of uterine fibroids in the health area of the Plana. Elaboration process: During 2017, a search was made of the best available evidence in the different sources of tertiary and secondary information: UpToDate, GPC, Societies ( SEGO) and databases (MEDLINE / PubMed) mainly. Clarifying summary tables of the diagnostic and treatment methods were prepared and management schemes were made. Conclusion: In the face of uterine fibroid the adequate diagnosis consists of physical examinat ion and ultrasound, the image added to the reproduction wishes and the symptomatology of the patient will determine the individualized therapeutic, medical or surgical approach, achieving an important improvement and recovery of an optimal biopsychosocial functioning.Introducción: Los miomas uterinos son tumores ginecológicos benignos con una prevalencia del 70%, aún más en la raza negra y con historia familiar. Son una causa habitual de sangrado menstrual anormal y síntomas derivados por presión. Su gran prevalencia hace que sean considerados un problema capital, afectando a la calidad de vida de las pacientes y provocando un impacto social y económico importante. Objetivos: Realizar una guía de práctica clínica que oriente en la toma de decisiones para el diagnóstico y tratamiento de las pacientes con sospecha o diagnóstico de miomas uterinos en el área de salud de La Plana. Proceso de elaboración: Durante 2017 se realizó una búsqueda de la mejor evidencia disponible en las distintas fuentes de información terciaria y secundaria: U pToDate, GPC, Sociedades (SEGO) y bases de datos (MEDLINE/PubMed) principalmente. Se elaboraron tablas resumen clarificadoras de los métodos diagnósticos y de tratamiento y se realizaron esquemas de manejo. Conclusión: Ante un mioma uterino el diagnóstico adecuado consiste en exploración física y ecografía, la imagen sumada a los deseos genésicos y la sintomatología de la paciente determinarán el planteamiento terapéutico individualizado, médico o quirúrgico, consiguiendo una mejoría importante y recuperaci ón de un funcionamiento biopsicosocial óptimo
