The importance of emotional disorders in adherence to a treatment plan among patients with heart failure with reduced ejection and heart failure with normal ejection fraction

The purpose of the study is to evaluate the influence of depression and anxiety on the compliance with the treatment plan among patients with a heart failure. Objectives of the study: 1. To assess the compliance of the treatment plan among patients who have chronic heart failure with anxiety symptoms. 2. To assess the compliance of the treatment plan among patients who have chronic heart failure with depression symptoms. 3. To compare the influence of depression and anxiety among the patients with heart failure with decreased left ventricular ejection fraction and patients with heart failure with normal left ventricular ejection fraction. Methodology of the study: 118 patients who have chronic heart failure with functional class III or IV (according to NYHA) were analysed. The patients' depression and anxiety were evaluated on the hospital scale of anxiety and depression. The compliance with the treatment plan was evaluated based on the Morisky-Green Self-Assessment Score and the Short Medication Questionnaire. Conclusions: 1. Patients who have chronic heart failure with depression symptoms 2 times more likely to forget to drink drugs (p = 0.000). 2. Patients who are depressed almost 3 times more likely to forget to take medication with themselves compared to patients who do not have depressive symptoms (p = 0.000). 3. The mean of depression was found to be statistically significantly higher those respondents who did not report discontinuation of treatment with doctors (p = 0.049). 4. The mean depression rate of HAD is statistically significantly lower in patients who know all or part of the drugs’ functions (p = 0.011). 5. The analysis showed that statistically significant higher anxiety level was reported by respondents who did not report discontinuation of treatment with doctors compared to those who report (p = 0.035). 6. Patients with anxiety symptoms nearly 3 times more often indicated that they had significant difficulty in paying for drugs than patients without anxiety symptoms (p = 0.014)
