Analysis of ventilator associated pneumonia due drug resistant acinetobacter baumannii

The aim of the study: To evaluate risk factors of ventilator associated pneumonia (VAP) caused by drug resistant A.baumannii among the MDR, XDR, PDR groups and find risk factors link with mortality. Objectives of the work: To determine the frequency of risk factors and of VAP caused by drug resistant A. baumannii among MDR, XDR and PDR groups. Also find out the relationship between them. Methodology: A retrospective analysis of medical records of 60 patients with VAP caused by drug resistant Acinetobacter baumannii in Lithuanian University of Health Sciences Intensive care clinics Central reanimation and intensive care unit during 2014–2015 has been carried out. Data were collected about: demographics, severity of illness, previous antibiotic use prior VAP onset, bed–days in hospital and ICU, outcomes. Statistical analysis was done using „IBM SPSS Statistics 20.0“. Results: There was MDR-VAP 13.3% (n=8), XDR-VAP 68.3% (n=41), PDR-VAP 18.3% (n=11) cases. The average score of SAPS II scale at ICU admission for MDR-VAP was 42.63±13.78, PDR-VP 49.00±13.45, XDR-VAP 48.71±12.81 (p=0.048). Prior to VAP carbapenems were used 50% MDR-VAP, 29.3% of XDR-VAP, 18.2% of PDR-VAP of cases (p=0.039). The length of hospitalization prior ICU admission of MDR-VAP was 0.25±0.46 d, XDR-VAP 2.85±4.89 d., PDR-VAP 5.64±6.65 d., (p=0.036).. Duration from hospital admission to start of mechanical ventilation was MDR-VAP 1.13±0.35 d., XDR-VAP 1.83±3.49 d., PDR-VAP 4.91±6.73 d. (p=0.013). Mortality in the MDR-VAP group was 62.5%, XDR-VAP 61.0%, PDR-VAP 72.7% (χ² = 0.591, p = 0.772). More often women (79.3%) (p = 0.013) have died than male.
