Enamel matrix derivate in periodontal regeneration: systematic review

Introduction: Untreated periodontitis leads to tooth loss. Successful periodontal treatment can be achieved by: 1. removal of etiological factor; 2. reduction of inflammation; 3. regeneration of lost periodontal tissue. There are several periodontal regenerative therapy methods, including guided tissue regeneration (GTR), bone augmentation (BG) and enamel matrix derivate (EMD) application. EMD is a purified acidic extract from 6 – month – old porcine enamel matrix . Aim: To carry out the options of enamel matrix derivate use for periodontal tissue regeneration. Methods: Scientific literature review was conducted by accessing to the data bases PubMed (www.ncbi.nlm.nih.gov/pubmed), Cochrane (http://www.cochrane.org/) and Wiley (http://onlinelibrary.wiley.com.ezproxy.dbazes.lsmuni.lt:2048/subject/code/DN13/titles). Articles were published in 2012 - 2017 years. Of 166 searches 27 selected articles met the criteria for analysis. Results: After systemic analysis of in vitro studies, the possitive effect of EMD on growing factors has been established. These factors stimulate new alveolar bone, root cementum and periodontal ligament synthesis. Moreover, they also have an angiogenic effect on periodontal blood vessels. In vivo possitive effect of EMD can be established by clinical examination and indicators of periodontal status (CAL, PD, GR) evaluation. Conclusion: Based on data analysis of in vitro studies and clinical periodontal evaluation in vivo, it has been concluded that enamel matrix derivate has a possitive effect on periodontal regeneration
