Main causes of fear of hypoglycemia in adults with type 1 and 2 diabetes, managements role

Hipoglikēmija ir viena no visbiežāk sastopamajām cukura diabēta terapijas komplikācijām. Novērots, ka cukura diabēta pacientiem, kas vairākkārt piedzīvojuši vidēji smagu vai smagu hipoglikēmijas epizodi, bailes no hipoglikēmijas sāk ietekmēt diabēta gaitu jeb pacienti sāk mēķtiecīgi uzturēt augstu asins plazmas glikozes līmeni, sakarā ar nepatīkām emocijām un sajūtām. Tas savukārt noved pie sliktas metabolās kompensācijas un paaugstināta komplikāciju attīstības riska. Tāpēc ir svarīgi saprast un izvērtēt hipoglikēmijas baiļu riska jeb nosakošos faktorus un rīkoties jau diabēta apmācību procesa ietvaros un arī turpmāku pacienta vizīšu laikā. Pētījuma mērķis bija novērtēt hipoglikēmijas biežumu gan 1., gan 2. tipa cukura diabēta pacientiem, izvērtēt hipoglikēmijas baiļu attiecīgos riska faktorus, izvērtēt hipoglikēmijas baiļu radītās sekas un sakarību ar dažādām cukura diabēta komplikācijām, cukura diabēta ilgumu, hipoglikēmiju biežumu un veidu. Lai izvērtētu cukura diabēta pacientu bailes no hipoglikēmijas, tika izmantota HFS-II Hipoglikēmijas baiļu anketa. Anketā ir iekļauti dažādi apgalvojumi par cukura diabēta pacientu darbībām un izjūtām, kas saistītas ar hipoglikēmiju jeb izvairīšanos no hipoglikēmijas. Apkopojot anketas kopējo punktu skaitu, iespējams noteikt hipoglikēmijas baiļu intensitāti, t.i., vairāk izteiktas bailes vai mazāk izteiktas bailes no hipoglikēmijas. Apkopojot iegūtos datus, tika konstsatēts, ka intensīvākas bailes no hipoglikēmijām tieši korelē ar smagas hipoglikēmijas epizodes piedzīvošanu, ar piedzīvoto hipoglikēmiju skaitu, kā arī nesenāk piedzīvotu hipoglikēmiju, savukārt dzimumam, izglītības līmenim, attiecību statusam , cukura diabēta tipam, saņemtajai antidiabēta terapijai un cukura diabēta apmācībai nav nozīmes hipoglikēmijas baiļu attīstībā. The aim of the study was to evaluate the incidence of hypoglycemia in type 1 and type 2 diabetic patients, to evaluate the relevant risk factors for hypoglycemia, to evaluate the effects of hypoglycemia, and to the association with various diabetes complications, duration of diabetes, the frequency and type of hypoglycemia. The HFS-II Hypoglycemia Fear Questionnaire was used to evaluate the fear of patients with diabetes mellitus from hypoglycemia. The checklist contains various allegations regarding the activities and feelings of patients with diabetes mellitus associated with hypoglycemia or avoidance of hypoglycemia. Summing up the total score of the questionnaire, it is possible to determine the intensity of the hypoglycemia fears, i.e., more pronounced fears or less pronounced fear of hypoglycemia.
