Focal segmental glomerulosclerosis (FSGS) is among the leading causes of end stage renal failure in adults (requiring dialysis and/or kidney transplantation) and a cause of nephrotic syndrome in children. The disease is considered primary when it is of idiopathic origin and of those, 30-50% exhibit recurrence (rec-FSGS) of the disease in the allograft following renal transplantation. The immediacy of FSGS recurrence (sometimes within a few hours of transplantation) suggests the presence of a circulating toxic factor that exerts its effects on glomerular epithelial cells (or podocytes).
