A school-based study of irritable bowel syndrome in college students in Beijing:prevalence and some relative factors

目的：调查北京某高校学生群体肠易激综合征( irritable bowel syndrome,IBS)患病情况及相关因素。方法采用横断面调查的形式进行。北京大学707名学生最终完成了封闭式自填问卷的调查。结果学生中IBS的患病率为16.7%,混合型( mixed IBS,IBS-M)比例最高,占48.5%(54/118),其次是腹泻型(diarrhea predominate IBS,IBS-D)占33.9%(40/118)。轻中度 IBS 占94.1%(111/118)、重度IBS占5.9%(7/118)。女性中IBS的患病率显著高于男性(20.2%,57/282 vs 14.4%,61/425, P=0.041)；医学专业和非医学专业学生IBS患病率分别为18.1%、15.3%(P=0.321)。以健康者为对照,IBS患者SCL-90(symptom checklist 90)中的躯体化症状、青少年生活事件量表(adolescent self-rating life check list,ASLEC)及考试焦虑量表(test anxiety scale,TAS)得分高,自测健康评定量表(self-rated health measurement scale,SRHMS)得分低(P<0.05)。结论本调查样本中大学生IBS的患病率为16.7%,女性患病率高于男性。学生IBS患者SCL-90躯体化症状总得分、ASLEC及TAS得分均显著高于健康对照组。Objective To investigate the prevalence and some relative factors of irritable bowel syndrome ( IBS) in college students in Beijing. Methods A cross-sectional study was carried out in Peking University. All of 707 students completed self-administered questionnaires. Results The prevalence of IBS in students was 16. 7% (118/707). Among the IBS patients, the subtype of IBS-M (mixed IBS) had the highest percentage, which was 48. 5% (54/118), followed by the IBS-D (diarrhea predominate IBS) (33. 9%, 40/118). In the IBS patients, 94. 1% (111/118) had mild-moderately IBS and 5. 9% (7/118) patients had severe IBS. For gender, the prevalence was higher in women than that in men (20. 2%, 57/282 vs 14. 4%, 61/425, P=0. 041). When it came to the specialty, the prevalence was higher in medical students than that in other major students (18. 1% vs 15. 3%), but there was no statistical difference (P=0. 321). Compared with healthy controls, the total scores of symptom checklist 90 (SCL-90) (somatization part), adolescent self-rating life check list (ASLEC) and test anxiety scale (TAS) were higher in IBS patients (P<0. 05). In IBS patients, the self-rated health measurement scale ( SRHMS) was significantly lower than that in healthy controls (P<0. 05). Conclusion The prevalence of IBS in students is 16. 7%. For gender, the prevalence in women is signifi-cantly higher than that in men.
