Les biopsies intestinales endoscopiques permettent des prélèvements à différents niveaux du duodénum et du jéjunum en vue de la détermination des activités enzymatiques. En endoscopie de routine, les prélèvements biopsiques doivent être effectués au niveau de l'angle de Treitz. A ce niveau, des différences importantes des activités muqueuses de la maltase, sucrase, lactase et phosphatase alcaline ont été observées par comparaison de sujets témoins contrôles et de malades coeliaques non traités. La technique permet également l'étude biochimique de lésions hétéromorphes ou « patchy lésions » chez les malades cœliaques
