An extremely rare clinical case of giant megaureter in an adult patient is described. Megaureter was revealed in examination of the abdominal cavity and retroperitoneal space during surgery for gynecological pathology. Surgical treatment was successful, resection of megaureter with plastic reconstruction of the left ureter with anastomosis "end- to-end" and "end-to-side" on introduced ureteral stents into the lumen was performed. Postoperative examination revealed the presence of megaureter on the right side and doubling of both kidneys
