Expression of protease activated receptors (PARs) 3 and 4 in idiopathic pulmonary fibrosis and idiopathic pulmonary arterial hypertension

Die idiopathische pulmonale Fibrose und die Idiopathische pulmonal-arterielle Hypertonie sind prognostisch ungünstige Krankheitsbilder unklarer Ätiologie, über deren histopathologische Mechanismen aktuell wenig bekannt ist. Bei beiden Krankheitsbildern scheint eine gestörte Gerinnungsituation an der Pathogenese beteiligt zu sein und an der Bildung von interstitiellem Lungenparenchym bzw. beim vaskulären Remodelling der pulmonalen Strombahn beizutragen. Die bisher entdeckten 4 Mitglieder der Familie der PARs sind ubiquitär auf nahezu allen Zellen des menschlichen Körpers vertreten und werden durch die Gerinnungsfaktoren verstärkt aktiviert. Eine mögliche Rolle von PAR-1 und PAR-2 bei der Vermittlung zellulärer profibrotischer bzw. des vaskulären Remodellings fördernde Effekte wurde bereits in früheren Studien gezeigt. In der vorliegenden Dissertation wurde die Expression von PAR-3 und PAR-4 in der Pathogenese der IPF bzw. IPAH untersucht. Die Ergebnisse zur Expression von PAR-3 zeigen, dass PAR-3 bei der IPF sowohl bei humanen Patienten als auch im Mausmodell der Bleomycin-induzierten Fibrose signifikant erhöht ist. Insbesondere in den hyperplastisch veränderten Typ-II Pneumozyten scheint eine verstärkte Expression im Krankheitsfall stattzufinden. Die Datenlage zur Expression von PAR-3 bei der IPAH ist nicht eindeutig. Die humanen Daten zeigen eine signifikante Expression vor allem in den hypertrophierten glatten Muskelzellen. Diese Signifikanz lässt sich jedoch im gewählten Mausmodell der Hypoxie-induzierten pulmonalen Hypertonie nicht bestätigen. Die Expression von PAR-4 ist bei beiden Krankheitsbildern nicht signifikant erhöht. Somit bleibt es Teil zukünftiger Forschungsarbeit, die genauen Signalkaskaden nachzuweisen, an denen PAR-3 bei beiden Krankheitsbildern beteiligt und inwieweit die erhöhte Expression relevant für die Krankheitsentwicklung ist. Weiterhin könnte im Tierversuch bei Mäusen in vivo mittels PAR-3 Knockout bzw. medikamentöse Inhibition die potentielle Auswirkung auf die Krankheitsentwicklung beobachtet werden.Idiopathic pulmonary fibrosis (IPF) and idiopathic pulmonary arterial hypertension (IPAH) are lung diseases characterized by a poor prognosis and unclear aetiology. Underlaying molecular mechanisms leading to the development of IPF and IPAH are barely understood. However, abnormalities in haemostasis seem to play a role in the pathogenesis of both diseases as they may influence pathologic remodelling of lung parenchyma and pulmonary vessels. Protease activated receptors (PARs) are found in almost all types of cells. Up to date four PAR receptors have been described. PARs are activated by proteases mainly belonging to the coagulation system. Recent studies show that PAR-1 and PAR-2 play a role in mediating profibrotic processes and vascular remodelling. This study focus on the characterisation of the expression of PAR-3 and PAR-4 in the lungs of IPF and IPAH patients as well as mice subjected to bleomycin-induced lung injury or exposed to hypoxia. Results show significantly elevated expression of PAR-3 in the lungs of IPF-patients and mice administrated with bleomycin, mainly attributable to hyperplastic alveolar type II cells (ATII). In IPAH increased expression of PAR-3 was predominantly observed in hypertrophic smooth muscle cells of pulmonary vessels, no such alterations were visible in the animal model of hypoxia-induced pulmonary hypertension. No changes were found in PAR-4 expression in both diseases. Future studies, employing PAR-3 knockouts and pharmacological inhibitors, are required to find out the possible role of PAR-3 in the pathogenesis of IPF and IPAH, especially in regard to its significantly elevated expression in AT II cells and hypertrophic smooth muscle cells of pulmonary vessels
