Purpose. Waldenstrom macroglobulinemia (WM) is a B-cell malignancy characterized by bone marrow infiltration of clonal lymphoplasmacytic cells, which produce a monoclonal immunoglobulin M (IgM). MYD88L265P mutation may be considered as a founder event because of it high frequency in WM. Whole-genome sequencing has revealed CXCR4 mutations (CXCR4Mut) in WM. CXCR4 is a G-protein-coupled receptor that promotes migration and activation of several pathways including RAS, Akt and NFKB. CXCR4 mutation has proved to be of critical importance in WM, in part due to its role as a mechanism of resistance to several agents of targeted therapy.
