In our clinic early stages analysis of laparoscopic roux-en-y gastric bypass and sleeve gastrectomy applied 50 patients with diagnosis of morbid obesity

Laparoskopik Roux-en-Y gastrik bypass (LRYGB) günümüzde sıkça kullanılan bariatrik bir prosedürdür ve laparoskopik sleeve gastrektomi(LSG) morbid obezite tedavisinde tek basamaklı bir prosedür olarak popülaritesi gittikçe artmaktadır. Bu çalışmada heriki yöntemin morbidite ve etkinliği açısından birbirine üstünlüğü olup olmadığını karşılaştırdık. Temmuz 2008 ile Mayıs 2010 arasında morbid obez tanısıyla operasyon önerilen 50 hastaya LRYGB (n:20) ve LSG (n:30) operasyonları hakkında bilgilendirilip karar verilen prosedür uygulandı. Hastaların ameliyat sürelerini, hastanede kalış sürelerini, morbidite, komorbiditelerini, sonuçlarını ve vücut kitle indeks (VKİ)'lerinin iki yıllık sürede azalmalarını inceledi. Heriki grup yaş, cinsiyet, VKİ ve komorbiditeler açısından karşılaştırıldı. Ortalama operasyon süresi LRYGB için 212dk. LSG için 98dk. olarak saptandı(p0.05). Ameliyat sonrası ilk yıl ve ikinci yıl için VKİ'lerindeki ortalama azalma oranı sırasıyla LRYGB için 15.58 kg/m2 ve 17,57 kg/m2, LSG için 16.05 kg/m2 ve 16,52 kg/m2 saptandı(p>0.05). Kısa dönemde her iki teknikte güvenli ve etkili birer yöntem olarak saptanmıştır. Ancak LSG ameliyat süresi olarak LRYGB'ye göre daha kısa olarak bulunmuştur. Dolayısıyla kısa süreli takip sonuçlarına dayanarak LSG, LRYGB yerine kullanılabilir alternatif bir yöntemdir.Laparoscopic Roux-en-Y gastric bypass (LRYGB) is one of the most widely used bariatric procedures today, and laparoscopic sleeve gastrectomy (LSG) as a single-stage procedure for the treatment of morbid obesity is becoming increasingly popular. In this study, we compared both techniques in order to establish whether there is any superiority of one over the other based on morbidity and effectiveness. From July 2008 to May 2010, 50 morbid obese patients with indication for bariatric surgery were assigned by patient choice after informed consent to either a LRYGB procedure (n=20) or a LSG procedure (n=30). We determined operative time, length of stay, morbidity, co-morbidity outcomes, and becoming less of body mass index (BMI) at 2 years postoperative. Both groups were comparable in age, sex, BMI, and co-morbidities. Mean operative time of LSG was 98 min while LRYGB was 212 min (p0.05). One year and two year after surgery, average becoming less of BMI in turn in order 15.58 kg/m2 and 15.57 kg/m2 in LRYGB and 16.05 kg/m2 and 16.52 kg/m2 in LSG (for each one p >0.05). In the short term, both techniques are comparable regarding safety and effectiveness, but LSG ise shorter than LRYGB. So in a short time results we can choise such a alternative technique LSG instead of LRYGB
