Malignant blue nevus is a rare, malignant, melanocytic tumor with a usually unfavourable prognosis. Here, we report two cases of malignant blue nevus occurring on the head. Both our patients had a history of progressive enlargement of a bluish lesion that had been present for several years. Histopathology revealed a dense, diffuse proliferation of bipolar dendritic and atypical, pleomorphic spindle-shaped melanocytes. One patient died of metastatic disease 16 months following excision of the primary tumor; the second patient died 36 months after discharge
