Expression of the costimulating molecules ILA (CD137) and ICOS (CD278) and its ligands in mast cells and T cells in the skin of psoriasis vulgaris patients

Erstmalig wurde in der vorliegenden Arbeit die Relevanz der kostimulatorischen Moleküle ILA-Rezeptor (ILA, induced by lymphocyte activation , CD137 in der humanen bzw. 4-1BB in der murinen Klassifikation) aus der TNF-Rezeptoren-Familie und ICOS-Rezeptor (ICOS, inducible costimulator , CD278) aus der CD28-Rezeptor-Familie bei der Hautkrankheit Psoriasis vulgaris in vivo belegt. Mittels immunhistochemischer Färbungen und Lichtmikroskopie sowie Doppelimmunfluoreszenz¬färbungen und konfokaler Lasermikroskopie wurde die Haut von 5 gesunden Probanden und sowohl die befallene wie auch unbefallene Haut von 5 Psoriasispatienten bezüglich der Expression dieser Moleküle untersucht. Wir konnten zeigen, dass in der Haut gesunder Probanden keine Expression des ILA- und ICOS-Rezeptors vorliegt, während interessanterweise bereits in der unbefallenen Haut des Psoriasiserkrankten ILA- und ICOS-Rezeptor exprimiert werden. Bei der Frage nach den exprimierenden Zelltypen konnte eine signifikante Hochregulation des ILA-Rezeptors auf Mastzellen sowohl in der unbefallenen als auch befallenen Haut des Psoriasiserkrankten, nicht aber in der gesunden Haut gezeigt werden. Die Expression von ILA-Rezeptor durch humane Mastzellen konnte damit erstmals in vivo gezeigt werden. Ebenso konnte auf T-Zellen in der Psoriasishaut die Expression von ILA-Rezeptor gezeigt werden. Jedoch war kein signifikanter Unterschied der ILA-Rezeptor-Expression auf T-Zellen in unbefallener und befallener Haut von Psoriasiserkrankten zu beobachten. ICOS-Rezeptor wurde von Mastzellen, T-Zellen und dendritischen Zellen der Psoriasishaut nicht signifikant exprimiert, wohl aber von Makrophagen. Für den ILA- und ICOS-Liganden zeigte sich in allen untersuchten Geweben, also sowohl bei gesunden Probanden wie auch in der unbefallenen und befallenen Haut des Psoriasiserkrankten, eine gleich starke, konstitutionelle Expression. Einer Aktivierung dieser Ligandensysteme dürfte in der Immunpathogenese der Psoriasis daher keine zentrale Bedeutung zukommen im Gegensatz zum ILA- und ICOS-Rezeptor. Wie bereits erwähnt, zeigt bereits die unbefallene Haut des Psoriasis-erkrankten eine Expression von ILA-Rezeptor und ICOS-Rezeptor. Dies könnte als Hinweis auf eine Dysregulation des Negativ-Rückkopplungs/ Feedback-Mechanismus dieser kostimulatorischen Systeme bei gleichzeitiger konstitutioneller Expression der Liganden gewertet werden. Das Zellaktivierungspotential stark ILA-Rezeptor-positiver Mastzellen in unbefallener Psoriasishaut dürfte ein wesentlicher Faktor für die Exzitabilität der unbefallenen Haut des Psoriasiserkrankten sein und damit eine mögliche Erklärung für das gut bekannte Köbner-Phänomen in der Psoriasis liefern. Wir identifizierten die Mastzelle als diejenige Zelle in der Psoriasis, welche den ILA-Rezeptor außer in befallener auch in unbefallener Haut des Psoriasispatienten exprimiert. Diese Daten legen eine zentrale Rolle der Mastzelle in der Zell-Zell-Interaktion bei der Initiierung der Psoriasisläsion auf Ebene des ILA-Rezeptors nahe.In this study the expression of the co-stimulatory molecules ILA receptor (ILA, "induced by lymphocyte activation", CD137 in humans and 4-1BB in mice) from the TNF receptor family and ICOS receptor (ICOS, "inducible co-stimulator", CD278) from the CD28 receptor family have been studied for the first time in psoriasis vulgaris in vivo. Skin from 5 healthy subjects and both lesional and non-lesional skin from 5 psoriasis patients have been studied applying immunohistochemical staining and light microscopy as well as double-fluorescence staining and confocal laser microscopy. Neither the ILA nor the ICOS receptor was found in the skin of healthy subjects, but both were positive in lesional and interestingly also in non-lesional skin of psoriasis patients. Concerning the cellular source we found a significant upregulation of ILA receptor on mast cells in both lesional and non-lesional skin of psoriasis patients compared with healthy subjects. Expression of the ILA receptor by human mast cells in vivo could be shown for the first time. Expression of the ILA receptor was also found on T cells in psoriatic skin, but there was no significant difference between T cells from non-lesional and lesional skin of psoriasis patients. ICOS receptor was not expressed by mast cells, T cells and dendritic cells. It was however expressed by macrophages in psoriatic skin. ILA and ICOS ligands showed an equally strong constitutional expression both in skin of healthy subjects and in non-lesional and lesional skin of psoriasis patients. Therefore activation of these ligand systems seems not to play a central role in the immunopathogenesis of psoriasis in contrast to ILA and ICOS receptor. While the ligands are expressed constitutionally, activation of the ILA and ICOS receptor could result from a dysregulation of negative feedback mechanisms within these costimulatory systems and could be involved in the immunopathogenesis of psoriasis. The potential for cell activation of mast cells which strongly express the ILA receptor in non-lesional psoriatic skin could play a causative role in the well known excitability of non-lesional skin of psoriasis patients. This may also explain the so called Koebner s phenomenon in psoriasis. We identified mast cells as the predominant cell expressing the ILA receptor in both non-lesional and lesional psoriatic skin. These results suggest a central role of ILA receptor expressing mast cells in cell-cell-interactions initiating the development of psoriasis lesions
