A Case of Acromegaly Associated with Thyroid Papillary Carcionma and Mammary Gland Intraductal Papilloma

Patients with acromegaly have an increased risk of developing cancer. We previously reported that patients with acromegaly showed an increased risk of colon cancer and polyps, and it is speculated that the risk of thyroid cancer is also elevated. In this study, we report a case of acromegaly associated with thyroid papillary carcinoma and mammary gland intraductal papilloma. A 65-year-old woman with diabetes mellitus was referred to our hospital due to her acromegalic features. Tests revealed that her basal plasma levels of growth hormone (GH) and insulin-like growth factor (IGF)-I had increased to 23.3ng/ml and 1,200ng/ml (+16.2 SD), respectively. Magnetic resonance imaging of the brain revealed the presence of a pituitary adenoma. She was given a 75g oral glucose tolerance test (OGTT) that showed no suppression of GH levels. The patient was diagnosed with acromegaly. Furthermore, thyroid ultrasonography revealed thyroid adenomatous nodules and papillary carcinoma. Several colon polyps, mammary gland intraductal papillomas and a giant nasal polyp were also revealed. After transsphenoidal surgery, the patient\u27s plasma GH and IGF-I levels normalized and her nadir GH level during OGTT decreased to less than lng/ml, suggesting that the acromegaly was cured. For patients with acromegaly there is an increased possibility of developing tumors as a complication. Patients with acromegaly should undergo colonoscopy and thyroid ultrasonography in particular to screen for complications
