Studies of ADL (Activities of Daily Living) and QOL (Quality of Life) -Related Factors in Medicine, Nursing, and Care Among the Aged and a 90-Year-Old and Over Population(Special Issue in Hornor of the Retirement of Professor Toshiko Takemiya at the Department of Neurology, Tokyo Women\u27s Medical University)

1) A specific score was assigned to each activities of daily living (ADL) item, and the scores were assessed for gender-related differences in 160 individuals (males, 24; females, 136: ages between 90 to 107 years, mean=93). The mean total score was higher in men. The ability to feed oneself without help had the highest score, with no gender-related difference. When limited to the male and female groups aged 90 to 95 years old and those able to feed themselves unaided, the scores for men were significantly higher. The men were superior in preservation of intellectual level and retention of satisfactory ADL. 2) We studied the type A behavior pattern in relation to longivity and life-style related diseases. The results suggested tendencies toward methodicalness and \u27going-my-way\u27 traits in the personalities of 33 aged people (mean age: 93 years old). In addition to methodicalness, business and hostility were selected as the personality traits of cerebrovascular disease (CVD) patients (mean age: 64.5 years old). 3) We examined accelerated plethysmography (APG) in 20 aged people (mean age: 93 years old; range: 90～106 years old). The APG-pattern (APG-P) revealed poor peripheral blood circulation, and they differed from person to person and from site to site. Disproportionate APG-P findings 2.5 years after the initial record, correlated with clinical events or poor outcome to some extent. Physicians who understand the pathogenesis of the patients diseases should play the role of team leader and educate other members of the medical staff, showing respect to their professions. This may contribute to successful aging
