Obturator hernia is comparatively rare and the preoperative diagnostic rate is low. This paper presents a case of obturator hernia diagnosed preoperatively by computed tomography (CT) and ultrasonography (US). An 88-year-old woman was admitted to the hospital because of abdominal pain and vomiting. A plain abdominal X-ray showed ileus. US revealed a round hypoechoic mass in the medial part of the right inguinal ligament. CT of the pelvic region showed a round low density mass between the right pubic and external obturator muscle. Under a diagnosis of strangurated obturator hernia, radical operation was performed. When this disease is suspected in an old patient with ileus, CT and US examination are very useful in making early diagnosis
