Surgical indications for cervical carotid stenotic lesions were established by international randomized control trials. We have treated 400 cervical carotid stenotic lesions with our carotid endarterectomy (CEA) method; a routine using a specially designed shunt system and a microscope. The shunt system was composed of a T-shaped silicone tube and clamps. The operative mortality and morbidity were 0% and 1.5%, respectively, which are acceptable levels in comparison with the results of international co-operative studies. Our method was applicable for higher cervical lesions extending to the upper C2 level. Multiple carotid lesions such as bilateral severe carotid stenoses were also safely treated.
