Where does Parkinson's disease begin in the brain?

Der dem idiopathischen Morbus Parkinson zugrundeliegende pathologische Prozeß benötigt Jahre zur Entwicklung seines vollen Schadenmusters. Er beginnt nichtin der Substantia nigra, sondern in umschriebenen Kerngebieten der Medulla oblongata. Die ersten Parkinsontypischen Eischlußkörper (Lewy Neuriten und Lewy Körper) innerhalb des Gehirns entstehen in den spindelförmigen viszeromotorischen (nichtcatecholaminergen) Nervenzellen des dorsalen Glossopharyngeusund Vagus Areals und in Projektionsneuronen der intermediär retikulären Zone. Vielfach werden sie von entsprechenden Veränderungen im Bulbus olfaktorius und im Nukleus olfaktorius anterior begleitet. Überdies können, bei intakter Substantia nigra, auch Projektionsneurone in Kerngebieten des ?gain setting? Systems (dencaudalen Raphekernen, dem gigagntozellulären Kern der Formation retikularis und dem Lokus coeruleus/subcoeruleus Komplex) Einschlußkörper entwickeln. Die vorliegende Studie beruht auf einer Analyse von 30 Fällen mit inzidenteller Lewy Körper Pathologie, von denen bei 26 Fällen keine Parkinsontypische Veränderungen in der Substantia nigra vorkamen. Außerdem wurden 58 altersund geschlechtsentsprechenden Kontrollen untersucht. Für den Nachweis der Einschlußkörper fanden immunocytochemische Verfahren mit einem Antikörper gegen das menschliche alphaSynuklein Anwendung. 26 Fälle und alle Kontrollen erwiesen sich als frei von Parkinsontypischen Veränderungen nicht nur in der Substantia nigra sondern auch in allen weiteren Prädilektionsstellen oberhalb der Substantia nigra, wie dem tuberomamillären Kern des Hypothalamus, dem interstitiellen Kern des diagonalen Bandes, dem Basalkern von Meynert, der Amygdala, dem zweiten Sektor des Ammonshorns und dem anteromedialen temporalen Mesokortex.The substantia nigra is not the induction site in the brain of the neurodegenerative process that underlies Parkinson’s disease. Instead, the results of this semiquantitative study of 30 autopsy cases with incidental Lewy body pathology indicate that Parkinson’s disease in the brain commences with the formation of the very first immunoreactive Lewy neurites and Lewy bodies in noncatecholaminergic visceromotor neurons of the dorsal glossopharyngeusvagus complex, in projection neurons of the intermediate reticular zone, and in specific nerve cell types of the gain setting system (coeruleussubcoeruleus complex, caudal raphe nuclei, gigantocellular reticular nucleus), olfactory bulb, olfactory tract, and/or anterior olfactory nucleus in the absence of nigral involvement. The topographical parcellation of the nuclear grays described here is based upon known architectonic analyses of the human brain stem and takes into consideration the pigmentation properties of a few highly susceptible nerve cell types involved in Parkinson’s disease. In this sample and in all 58 ageand gendermatched controls, Lewy bodies and Lewy neurites do not occur in any of the known telencephalic Parkinson’s disease predilection sites (hippocampal formation, temporal mesocortex, proneocortical cingulate areas, amygdala, basal nucleus of Meynert, interstitial nucleus of the diagonal band of Broca, hypothalamic tuberomamillary nucleus)
