Urinary-total-hydroxyproline was measured in 8 cases of primary hyperparathyroidism and 11 cases of chronic renal failure, 6 showing evidence of secondary hyperparathyroidism. Bone involvement was assessed in bone-biopsy specimens and on x-ray appearances. Urinary hydroxyproline was elevated in 7 of the cases of primary hyperparathyroidism in proportion to the degree of bone involvement. In one case with normal urinary hydroxyproline, bone histology was not definitely normal. In 5 of the 6 cases of chronic renal failure, urinary hydroxyproline was elevated, but values did not correspond closely to the degree of bone disease assessed on x-ray appearances
