The ossification of the medial clavicular epiphysis in forensic age diagnostics to determine the completion of the 21st year : sonography and morphological causes of differences to computer tomography and macroscopy

Die Intention dieser Arbeit war die Untersuchung der Frage ob die sonografische Beurteilung des Ossifikationsstadiums der medialen Klavikulaepiphyse die Vollendung des 18. bzw. 21. Lebensjahres nachweisen kann. Sie setzt sich aus drei Teilen zusammen: Zum einen wurde die Verknöcherung der medialen Schlüsselbeine beidseits bei 414 Individuen beiden Geschlechts im Alter zwischen 14 und 26 Jahren sonografisch bewertet. Das Stadium 4, welches der vollständigen epiphysären Fusion entspricht, wurde in allen untersuchten Altersgruppen beobachtet. Es konnte geschlussfolgert werden, dass bei 12,1 % der minderjährigen Probanden dieses Stadium falsch positiv vergeben worden ist. In einer vergleichenden Untersuchung in der Beurteilung der medialen Klavikula zwischen Sonografie, Computertomografie und Makroskopie an fünf männlichen Verstorbenen im Alter zwischen 15 und 28 Jahren konnten Ursachen für diese falsch positive Stadienbestimmung ermittelt werden: Normvarianten können ein Stadium 4 imitieren, obwohl sich das Schlüsselbein tatsächlich noch in einem frühen Stadium der Entwicklung befindet, vorhandene Reste der Epiphysenfuge vor allem im dorsokaudalen Bereich können aufgrund des eingeschränkten sonografisch einsehbaren Untersuchungsbereich übersehen. Des Weiteren können sonografische Bildartefakte tatsächlich nicht vorhandene Epiphysenfugen oder auch knöcherne Ossifikationszentren imitieren. Insgesamt stimmten die sonografisch ermittelten Ergebnisse in sieben von zehn Fällen nicht mit den computertomografisch bzw. makroskopisch ermittelten überein. In einer Auswertung der sonografischen Morphologien konnten einzelne Konfigurationen der medialen Klavikula identifiziert werden, deren Vorhandensein nicht zwangsläufig einer tatsächlich vorliegenden vollständigen Fusion entsprechen muss. Eine genaue Unterscheidung der Stadien 1 und 4 gelingt anhand der Sonomorphologie jedoch nicht. Nach den in dieser Arbeit vorgestellten Ergebnissen kann die Sonografie nicht die Computertomografie in der Beurteilung der medialen Klavikulaepiphyse im Rahmen der forensischen Altersdiagnostik als Goldstandard ablösen.The intention of this work was to study the question whether the sonographic assessment of the ossification of the medial clavicular epiphysis can prove an age of 18 or 21 years. It consists of three parts: First, the ossification of the medialen clavicle was evaluated sonographically on both sides at 414 individuals of both sexes aged between 14 and 26 years. The stage 4, which corresponds to the complete epiphyseal fusion, was observed in all age groups. Based on a literature review it could be concluded that in 12,1 % of the underage volunteers this stage was assigned false positive. In a comparative study in the evaluation of the medial clavicle between ultrasound, computed tomography and macroscopy of five male dead aged between 15 and 28 years causes of these false positive staging could be identified: normal variants can mimic a stage 4, although her epiphysis is in fact at an early stage of development, existing remains of the epiphyseal plate, especially in the dorsocaudal area, may be overlooked due to the restricted sonographically field of view. Furthermore, sonographic image artifacts can actually imitate nonexistent epiphyseal plates or bony centers of ossification. Overall the sonographic results obtained in seven out of ten cases did not match with the CT or macroscopy. In an review of the sonographic morphology some configurations of the medial clavicle could be identified, which presence does not necessarily correspond to an actually existing fusion. However, a precise distinction of stages 1 and 4 is not possible on the basis of sonomorphology. According to the results presented in this work, ultrasonography can not replace computed tomography as the goldstandard in the assessement of the medial clavicular epiphysis in the context of forensic age estimation
