We agree with many of the points made by Lurbe and Redon,1 which were similar to those raised in our own article. Certainly, the ability to assess central blood pressure (BP) noninvasively has increased our understanding of isolated systolic hypertension (ISH) in young people. However, rather than providing any mechanistic information, assessment of central BP simply highlights that ISH in young people cannot be attributed to exaggerated amplification of a normal central BP, but, instead, is associated with elevated central and brachial systolic BP. In this regard, measurements of cardiac output and stroke volume are more informative and could, potentially, be used to stratify risk in young individuals in the future
