Vécu des femmes suivies au diagnostic prénatal pour hyperclarté nucale avec bilan fœtal négatif

The discovery of an ultrasonographic presenting sign during the first trimester of pregnancy modify all the adaptive psychic process that the futures parents try to find out. Indeed, the announcement of a increased nuchal translucency appears early during the pregnancy and affects severely the parents. The objective of this study is to know the anxiety level created by the complementary examinations made during the prenatal diagnosis and to identify afterwards the elements which can lead to a difficult pregnancy experience. Twenty semi-structured interviews have been done to the CPDPN of Port Royal maternity hospital (thirteen prenatal interviews and seven postnatal interview). We have demonstrated that the physician’s attitude during the announcement seems to be appropriate but the information given to some couples can be imprecise and abstract. The uncertainty of the prognosis can lead sometimes to an immediate disinterest of the pregnancy which can be irreversible. Usually, it gives way to hope, even if it keeps time suspended. The announcement of reassuring results permits to the women to be involved in their pregnancy and permits to blur the initial uncertainty. It’s only at the end of the medical examinations, when the doubts disappear, that the patients really understand the stakes of an ultrasonographic presenting sign and especially that it wasn’t a diagnosis, but a risk identification. This women, despite a particular pregnancy, succeeded in projecting peacefully into the future and they didn’t describe having developed more postpartum psychic disorders. The suggestions that can be made as a result of this study: to give clear, homogenous and adapted information to each couple. Their temporality has to be respected to let the stunning time diminish. It seems to be an advantage for the couples that the specific information for a increased nuchal translucency care are given in a different place, in a different time, and by another person from the medical staff.La découverte d’un signe d’appel échographique au premier trimestre bouleverse tout le processus psychique d’adaptation que les futurs parents cherchent à établir. En effet, l’annonce d’une hyperclarté nucale apparait très tôt dans le déroulement de la grossesse et perturbe fortement les représentations parentales. L’objectif de cette étude est de connaitre le niveau d’anxiété générée par les investigations complémentaires réalisées au diagnostic prénatal et d’identifier les éléments à postériori qui peuvent engendrer un vécu difficile de la grossesse. Vingt entretiens semi-directifs ont été réalisés au CPDPN de la maternité Port Royal (treize entretiens prénataux et sept entretiens postnataux). Nous avons alors montré que l’attitude de l’échographiste au moment de l’annonce semble être adaptée mais les informations délivrées trop imprécises et abstraites pour certains couples. L’incertitude du pronostic peut parfois entrainer un désinvestissement immédiat de la grossesse pouvant être irréversible. Elle laisse le plus souvent place à l’espoir même si elle met le temps en suspension. L’annonce des résultats rassurants permet aux femmes de s’investir de nouveau dans leur grossesse et à l’incertitude initiale de s’estomper. C’est seulement à la fin des investigations médicales, quand les doutes disparaissent, que les patientes intègrent véritablement les enjeux d’un signe d’appel échographique et notamment qu’il ne s’agissait pas d’un diagnostic mais de l’identification d’un risque. Ces femmes, malgré une grossesse particulière, réussissent à se projeter sereinement dans le futur et n’ont pas décrit avoir développé plus de troubles psychiques du post-partum. Les propositions pouvant être apportées à la suite de l’étude : délivrer des informations claires, homogènes et adaptées à la singularité de chaque couple. Le respect de leur temporalité est nécessaire pour laisser le temps à la sidération de s’estomper. Il semble bénéfique pour les couples que les informations spécifiques à la prise en charge d’une HCN soient données dans un temps et un lieu différent, ainsi que par un autre professionnel que l’échographiste
