Patiënttevredenheid en functie van de knie bij status na reconstructie van de posterolaterale hoek

Background: The posterolateral corner is one of the main stabilizers of the knee. Injuries to the posterolateral corner are not common and mostly difficult to diagnose. It is often seen in a multiligament injured knee whereby anterior cruciate ligament and/or posterior cruciate ligament are also injured. Untreated injuries of the posterolateral corner can lead to substantial patient morbidity and functional instability. Aim: In this study the patient satisfaction and function of the knee was investigated after reconstruction of the posterolateral corner. Design: retrospective cohort study Methods: Nineteen persons, who underwent a reconstruction of the posterolateral corner in the period 2000 to 2011, were included in this study. The group existed of fourteen men and five women with a mean age of 33 years. The mean follow-up was 4,67 years. The patientsatisfaction was measured with the subjective IKDC-questionnaire and the function of the knee was measured with knee examination scored with the objective IKDC-scoringssystem. Results: In this study 95% scored normal or nearly normal on the Dialtest (exorotatie) after the reconstruction. Preoperative scored 84% abnormal on the Dialtest. All persons showed preoperative varusinstability. Postoperative 63% scored normal or nearly normal on varusinstability. The preoperative subjective IKDC-scores improved significant after a reconstruction, with an average increase of 17 points. Conclusion: A reconstruction of the posterolateral corner with a 2-graft technique results in normal or nearly normal posterolateral kneestability and improves the patientsatisfaction significantly.
