Molecular Definition of Burkitt Lymphoma Two types of malignant lymphoma are the main subject of this thesis: Burkitt lymphoma and diffuse large B-cell lymphoma (DLBCL). The distinction between these two lymphoma types has a major impact on treatment and prognosis for the individual patient. This distinction is especially difficult in a group of gray zone lymphomas with characteristics of both entities. In this thesis we tried to better define BL with a number of current and newer techniques. Our epidemiological study in The Netherlands showed that BL typically involves young boys and its incidence gradually increases in elderly patients. We showed that BL has a typical morphology, immunophenotype and almost always harbors a MYC translocation, but that an overlap exists with a subset of DLBCL cases. Next, we determined if other/newer techniques could improve the distinction between BL and DLBCL. Based on micro-array experiments we show that BL harbors a specific and homogeneous gene expression pattern. Using this technique the gray zone could be reduced, however it is not yet generally available in daily clinical practice. Based on DNA array-CGH experiments we showed that BL harbors only few genetic aberrations in addition to the classic MYC translocation. This was confirmed in a large retrospective analysis of all previously published cytogenetic data. This thesis shows that we could reduce the gray zone between BL and DLBCL. However, the problem could not yet be fully solved on an individual patient basis. The work presented has made a contribution to the most recent version of the WHO lymphoma classification, which is used worldwide. De moleculaire definitie van het Burkitt lymfoom Twee typen van lymfeklierkanker (maligne lymfoom) staan centraal in dit proefschrift: het Burkitt lymfoom (BL) en het diffuus grootcellig B-cel lymfoom (DLBCL). Onderscheid heeft grote consequenties voor behandeling en prognose. Helaas bestaat er een grijze zone van probleemgevallen met karakteristieken van beide lymfomen. In dit proefschrift hebben wij getracht het BL beter te definiëren door toepassing van een aantal bestaande en nieuwe technieken. Ons epidemiologisch onderzoek in Nederland liet zien dat het BL een relatief hoge frequentie heeft bij jongens met daarnaast een geleidelijke toename in oudere patiënten. Verder bleek het BL maar ook een klein deel van de DLBCL een typisch eiwit expressie profiel en een typische chromosomale translocatie te bevatten. We onderzochten vervolgens of toepassing van nieuwe technieken het onderscheid verbetert. Middels genexpressie array onderzoek bleek het BL een kenmerkende set genen tot expressie te brengen. Het aantal probleemgevallen lijkt hiermee zeker te kunnen worden gereduceerd, maar helaas is deze techniek nog niet beschikbaar voor de dagelijkse praktijk. Middels DNA array-CGH onderzoek bleek het BL slechts weinig genetische afwijkingen te bevatten. Dit laatste werd bevestigd in een grote retrospectieve analyse van alle in de literatuur gepubliceerde gevallen. Dit proefschrift laat zien dat we de grijze zone tussen het BL en DLBCL beter hebben kunnen herdefiniëren en reduceren, maar op het individuele patiënten niveau (nog) niet volledig kunnen elimineren. Ons onderzoek heeft bovendien bijgedragen aan concrete aanpassingen in de World Health Organization classificatie voor maligne lymfomen die in 2008 werd gepubliceerd en in de hele wereld gebruikt wordt.
