The implementions of case management in addictological servises in the Czech Republic

This bachelor thesis discusses the use of case management in addictology services and its applicability in Czech Republic. The main goal of the thesis is to study the application of case management in selected services. The thesis consists of two parts, which includes history of case management, basic characteristics of the case management models and the principles of effective case management by Rapp, and processing and evaluation of data collected during individual half-structuralized interviews with members of service providers respectively. Interviews aren't part of the thesis. The pieces of information are processed based on comparison of the principles of effective case management. Outcomes of this work cannot be transferred to any other service providers, because researched service providers created individualized processes. The use of case management in Czech Republic is not completely settled and does not have stable foundations in medical practice yet
