FAKTOR-FAKTOR RISIKO YANG BERPENGARUH TERHADAP KEJADIAN KATARAK SENILIS (STUDI KASUS DI KOTA SEMARANG DAN SEKITARNYA)

Background : Seniles Cataract is still a health problem in community. Based on the Survey conducted in 1996, cataract is the main cause of blindness, where as the prevalence of cataract 0,78% of the prevalence of blindness 1,5%. The objective of this study is to understand the risk factors influencing the incidence of senilis cataract. Method : Case — control. The case is seniles cataract patients in ophthalmologic health center in Semarang, patients who got mass cataract surgery held by Panti Wilasa Hospital July 11-12, 2003 and that held by William Booth Semarang Hospital August 23, 2003. The control is non cataract patients in ophthalmologic health centre from January until August 2003. There are 72 cases and there are 72 patients as control. The data are taken from medical record, interview and measurement. Result : Bivariate analysis show all determinant variables : age over 66 years old, female, lack of education, low income, smoking habit, outdoor work, animal protein consuming pattern — twice-three times a week or irregular, nabatic protein consuming pattern twice — three times a week or irregular, vegetable consuming pattern twice-three times a week or irregular, drinking cataractogenic drugs are significant to the incidence of senilis cataract. The multivariate analysis show proved variables influencing simultaneously are : age over 66 years old, OR: 9,0 (CI: 2,9-27,6); Animal protein consuming pattern twice-three limes a week or irregular OR: 7,0 (2,3-20,4); Outdoor work OR: 6,7 (CI: 3,2-20,2); Smoking habit OR: 5,8 (CI: 1,8-18,8); Lack of education OR: 4,2 (CI: 1,1-16,3); Nabatic protein consuming pattern twice-three times a week or irregular OR : 5,2 (CI: 1,7-15,2). Conclusion : There are six variables proved influencing the incidence of senilis cataract, they are : age over 66 years old, animal protein consuming pattern twice-three times a week, outdoor work, smoking habit, lack of education and nabatic protein consuming pattern twice-three times a week. Recommendation : There should be some efforts to delay the incidence of seniles cataract among others nutrition promotion, consuming animal and nabatic protein, healthy working condition by avoiding working under sun exposure. The model preventions are screening model and further research on cataract. Latar belakang : katarak senilis masih menjadi masalah kesehatan masyarakat, menurut Survei pada tahun 1996, katarak merupakan penyebab utatna kebutaan, dimana prevalensi buta katarak 0,78% dad prevalensi kebutaan 1,5%. Penelitian bertujuan untuk mengetahui faktor risiko yang berpengaruh terhadap kejadian katarak senilis. Metode : kasus — kontrol. Kasus adalah penderita katarak senilis di Balai Kesehatan Indera Masyarakat Semarang, dan peserta pada operasi katarak massal di RS Panti Wilasa Citarum Semarang, 11-12 Juli 2003 dan RS William Booth Semarang 23 Agustus 2003. Kontrol adalah penderita bukan katarak senilis di Balai Kesehatan Indera Masyarakat Semarang antara bulan Januari sampai bulan Agustus 2003. Jumlah kasus sebanyak 72 orang, sedangkan kontrol 72 orang. Data diperoleh dad catatan medis, wawancara dan pengukuran Hasa : Analisis bivariat menunjukkan bahwa seluruh variabel determinan yang diteliti : umur > 66 tahun, jenis kelamin perempuan, pendidikan rendah, pendapatan rendah, kebiasaan merokok, pekerjaan di luar gedung, pola konsumsi protein hewani 2-3 kali per minggu atau tidak tenth, pola konsumsi protein nabati 2-3 kali per minggu atau tidak tentu, pola konsumsi sayuran 2-3 kali per minggu atau tidak tenth, kebiasaan minum obat-obatan kataraktogenik berhubungan bennakna dengan kejadian katarak senilis. Hasil analisis multivariat menunjukkan variabel yang terbukti berpengaruh secara bersama-sama adalah : umur > 66 tahun, OR = 9,0 (CI : 2,9 — 27,6); pola konsumsi protein hewani 2-3 kali per minggu atau tidak tenth OR = 7,0 (CI : 2,3 — 20,4); pekerjaan di luar gedung OR = 6,7 (CI : 3,2 — 20,2); kebiasaan merokok OR = 5,8 (CI : 1,8— 18,8); pendidikan rendah OR = 4,2 (CI:1,1-16,3); pola konsumsi protein nabati 2-3 kali per minggu atau tidak tenth OR=5,2 (CI:1,7— 15,2). Simpulan : terdapat enam variabel yang terbukti berpengaruh terhadap kejadian katarak senilis, yaitu : umur > 66 tahun, pola konsumsi protein hewani 2 — 3 kali per minggu, pekerjaan di luar gedung, kebiasaan merokok, pendidikan rendah dan pola konsumsi protein nabati 2-3 kali per minggu. Saran : perlu adanya tindakan upaya penundaan katarak senilis, berupa penyuluhan gizi : konsumsi protein hewani dan nabati, kesehatan kerja seperti menghindari bekerja langsung di bawah sinar matahari. Model upaya pencegahan, model skrining sena faktor prediksi buta katarak perlu penelitian iebih lanjut
