SUMMARY

We describe a case of primary T cell lymphoma of the liver developing in a patient with Felty's syndrome (FS). We discuss the possible relationship of the two conditions with particular reference to liver disease in FS, and the role of the T cell in RA. FELTY'S syndrome (FS) is a variant of RA occurring in about 1 % of patients, characterized by severe long-standing arthritis, unexplained neutropenia, and sple-nomegaly [1]. Of particular interest is that over 90 % of these patients are HLA-DR4 positive, compared to 70 % of RA patients, and 30 % of controls [2]. Since T cells are important in mediating RA, and HLA mol-ecules are involved in the presentation of antigen to T cells, research is in progress to investigate any bias in T cell receptor (TCR) usage in patients with RA, with the aim of identifying pathogenic T cells [3, 4]
