Bleeding into individual cysts or hematuria is a common complication of polycystic kidney disease (PCKD) (1). However, to date no one has reported bleeding from the kidney into the peritoneal cavity with consequent bloody effluent in a patient with PCKD on continuous ambulatory peritoneal dialysis (CAPD). While on holidays abroad, a 39-year-old man with PCKD, who had been on CAPD for five months, developed severe right loin pain.
