Is there a patient in the text? A discourse study of patient imaging in case reports from British and American medical journals

Wydział Neofilologii: Instytut Filologii AngielskiejThe present dissertation seeks to examine critically how patients are portrayed in professional medical texts. The research question is whether patients are presented as subjects or objects of medical study and practice, or, in other words, whether they are imaged as whole persons and experiencing individuals in the descriptions of diagnosis and treatment. To this aim, a corpus of 56 medical case reports from British and American journals has been analysed paying special attention to the textual presence or absence of patients in the accounts of diseases and medical procedures, and possible explanations for these modes of reference. In the theoretical part, both historical and modern contexts in which case reports evolved and are used respectively have been discussed as factors shaping this genre of medical discourse. In the first stage of the empirical part, the quantitative and qualitative analyses of the means of patient imaging have been conducted. The study allowed to identify two types of discourse used to describe patients, i.e. the patient-centred discourse and the objectifying discourse. It has been demonstrated that in some sections there is a relationship between the type of information conveyed and the choice of the linguistic means used to describe the patient. In the second stage of the empirical part, the reception of the construction of the patient persona in the texts under examination has been studied. To this aim, a questionnaire has been administrated among the students of medicine in order to determine whether the interpretation of the texts from the linguist’s point of view will coincide with the interpretation of other readers.Celem niniejszej rozprawy doktorskiej było zbadanie dyskursu dotyczącego pacjentów w medycznych opisach przypadku pochodzących z brytyjskich i amerykańskich czasopism. Praca ta miała na celu odpowiedź na pytanie czy pacjent prezentowany jest jako podmiot czy przedmiot medycznych badań i leczenia, oraz czy autorzy uwzględniają w tekstach odczucia, jakich doświadcza pacjent w trakcie choroby. Dysertacja składa się z sześciu rozdziałów – czterech teoretycznych i dwóch empirycznych. Celem części teoretycznej było naświetlenie historycznego i współczesnego kontekstu, w jakim powstał i jest używany medyczny opis przypadku jako czynników kształtujących ten gatunek medycznego dyskursu. Pierwsza część badań empirycznych zawiera ilościową i jakościową analizę środków językowych służących obrazowaniu pacjenta. Przeprowadzone analizy umożliwiły wyodrębnienie dwóch typów dyskursu używanego w badanych tekstach: dyskursu skupionego na pacjencie oraz dyskursu uprzedmiatawiającego pacjenta. Stwierdzono między innymi, iż w niektórych sekcjach opisów przypadku istnieje zależność między przekazywanym typem informacji a wyborem środków językowych opisujących pacjenta. W pierwszej części badań empirycznych skupiono się na konstrukcji obrazu pacjenta w tekście medycznym, natomiast w części drugiej zbadano recepcję tego obrazu. W tym celu przeprowadzono ankietę wśród studentów medycyny, która sprawdziła czy interpretacja tekstów z perspektywy językoznawcy jest zbieżna z interpretacją czytelników bez przygotowania językoznawczego
